Description
(i)This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. (ii) RFQ F3M3C16128A100 is a combined synopsis/solicitation. This combined synopsis/solicitation is issued as a request for quote (RFQ). Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-25. It is the contractor?s responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm. (iv) This procurement is being issued as 100% Small Business Set-Aside. North American Industrial Classification Standard: 238210; Small Business Size Standard: $13,000,000. (v) CLIN (0001) Emergency Repairs needed on AFCT Corrosion Control Facility system (3 new fan motors and 2 magnetic contactors plus labor) in accordance with the Statement of Work. (vi) STATEMENT OF WORK Location: Corrosion Control Facility (B750) Inspector: Mr. Barney Lee 43 CES/CEOE, 1.910.394.2823 Date: 15 May 2006 SCOPE: Contractor shall provide all material and labor to install the following: 1. (1) Fan motors (Manf: Baldor, Cat#: M4314T, Spec#: 14C051X279H1, Frame: 364T) or approved equal.. 2. (1) Magnetic Contactors (Manf: Schneider electric/Square D, PN: LC2D115) or approved equal.. 3. Additionally the contractor shall furnish 2 motors (item no.1), 1 additional Magnetic contactor (item no.2) for delivery to Mr. Logan 43CES/CEOFH The exhaust fan units are located on the roof of the CCF. A crane may be required to perform the work. It is strongly advised that all potential bidders arrange for a site visit prior to submitting a bid. Contractor shall provide his own lift or crane to perform the work. Contractor shall also be responsible for providing a qualified electrician to make any final electrical connections under this SOW. Contractor shall also be responsible for the following: 1- Removing the fan from the roof to make repairs and placing it back on the roof when complete if necessary. 2- Disposal of all construction materials at base designated site. 3- Any safety equipment needed for repair work of the fan on the roof itself. 4- Greasing/lubing of fan. 5- Any damage to curb or roof from the removal of the fan. 6- All materials required to do the work. 7- Verification that the units are functioning properly with VFD?s when work is completed. Contractor shall fill out and return Pope AFB IMT 3140 (consent for background check) for all workmen assigned to the project immediately after contract award for obtaining base passes. PERFORMANCE TIME: The delivery time for materials is 14 days or sooner. Installation must be executed and completed within 3 days of material delivery. WORKMANSHIP: Work must conform to all local, state, and federal codes and requirements that are common to this trade. SITE AVAILABLITY: Work hours shall be between the hours of 0700 and 1700 Monday through Friday except Holidays. WARRANTY: All material and parts shall be warranted for a period of at least one year. All labor shall be warranted for a period of 30 days from acceptance. A Site visit is scheduled for 22 May 2006 @ 1000 am and we will meet at the Reilly Gate entrance to Pope Air Force Base. (viii)Period of Performance 14 days for delivery of materials and 3 days to execute and complete installation. (ix) The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. (x) The provision at FAR 52.212-2, Evaluation ? Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price and past performance. (xi) Offerors must include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications -- Commercial Items, with it?s offer. (xii) The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xiii) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are considered checked and are applicable to this acquisition: (13-19), (24), (29 c1 and c2). (xiv) The following additional provisions and clauses apply to this acquisition: FAR 52.232-33; DFARS 252.204-7003; DFARS 252.204-7004 (Alt A); DFARS 252.243-7001 (Fixed Price); DFARS 252.212-7001; (The following sub-paragraphs are considered checked and are applicable to this acquisition: DFAS 252.225-7001 and DFARS 252.247-7023.); DFARS 252.232-7003. Award can only be made to contractors registered in the Central Contractor Registration web page, http://www.ccr.gov. (xv) N/A.
