Lawton USPHS Indian Hospital has a requirement for a 5-year lease of a Picture Archiving Communication System (PACS) and digital radiology equipment as described herein and in following postings 246-06-Q-0034B and C. DESCRIPTION/SCOPE The contractor shall provide a lease agreement that includes all personnel, equipment, IHE Statements, DICOM Conformance Statements, software, hardware, tools, material, supervision, training, licensing, other items and services necessary to perform integration, testing and installation and initial Administration of a PACS and DRS at the USPHS Lawton Service Unit, 1515 Lawrie Tatum Road, Lawton, Oklahoma, 73507. The Objective of this acquisition is to obtain a 60 month lease agreement of a proven state of the art computer based Picture Archiving and Communication System (PACS) Network and replacement of a time-worn radiographic room with a Digital Radiographic System (DRS). The DRS generates X-rays, captures and converts X-rays to digital data, reconstructs and processes an image, then sends the image to the selected destinations via the PACS network. Types of services which are going to be run on the network include but are not limited to; file services, application services, database services, message/mail services and print services. The Agreement will include commercial equipment warranties and extended warranties as they apply and advanced service training. The system must be retroactively fitted into an existing hospital and satellite clinic facilities. This is the first such installation in this Service Unit. Overall program objective: Develop a secure Digital Imaging and Communication in Medicine (DICOM) Network that integrates all of the following: -Modalities of a Radiological System i.e. Computer Radiology (CR), Digital Radiology (DR), Ultrasound (US), Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Imagers, Digitizers. -Software i.e. DICOM, Drivers, HIS/RIS, HL7, Broker, Tele Radiology, World Wide Web. -Hardware i.e. Network Cards, Switch/Hub, Cable, Media -Devices i.e. Disk drives, Printers, Mice, Modems, Monitors, View Stations -Nodes i.e. Servers, Gateways/Routers, Archives, Redundant Array of Inexpensive Devices (RAID), Short Term Storage, Radiologist Station, Clinical Station, Patient Entry Station, Administrative Station, Voice Activated Dictation and Bar code System A DICOM Communication system such as Combination, or Hybrid Networking configuration with high speed, high capacity network media developed in accordance with Open System Interconnection (OSI) Model and IEEE 802 Specification Enhancements will be needed. Facility Background: The Lawton Service Unit includes the Lawton Indian Hospital (LIH), the Anadarko Indian Health Center (AIHC), and the Carnegie Indian Health Center (CIHC). Lawton Indian Hospital has 42 beds and a staff of 26 physicians who attend over 115 deliveries and perform approximately 900 surgical procedures per year. This full-service facility offers inpatient care including general surgery, obstetrics and gynecology, internal medicine, and pediatrics, as well as outpatient services in medicine, dentistry, pharmacy, radiology, laboratory, nursing, optometry, and audiology, to name a few. There is also a community health staff of nurses, educators, social workers, and sanitarians. Over 2100 adult and pediatric admissions annually, combined with almost 65,000 outpatient visits to make this a busy health care facility. Also in the service unit is a satellite clinic located in Anadarko, Oklahoma. Anadarko is a small rural community of 6,500 about 64 miles from downtown Oklahoma City. With a staff of 3 physicians and one physician assistant, the Anadarko health center handles nearly 35,000 outpatient visits each year. The 20,000-square-foot facility performs all of the usual outpatient and community services. Its clinical laboratory, its radiology department, and its dental clinic are all currently modernizing as well. Another satellite clinic is located in Carnegie, Oklahoma; a small rural community of 1,500 is about 85 miles from Oklahoma City, Oklahoma. The clinic serves approximately 10,000 visits per year. This clinic is a national test site for the Electronic Health Record. Description of specific tasks: It is understood by all parties that this objective is developed to establish a lease contract subject to availability of funds. The Period of Performance of this lease will be 60 months (the ?Lease Term?) consisting of a base period and renewal periods as described below: Base Period-Date of Acceptance through month 12 of Lease Term Renewal Period 1-Month 13 through month 24 of the Lease Term Renewal Period 2-Month 25 through month 36 of the Lease Term Renewal Period 3-Month 37 through month 48 of the Lease Tern, Renewal Period 4-Month 49 through month 60 of the Lease Term Funds of the Fiscal Year Obligation described above for the base period are obligated with the issuance of the Lease agreement, and the Government shall provide the Contractor with a funded modification to the agreement for each successive renewal period as soon as practicable, but in no event later than (10) business days after the Government receives notice of availability of fiscal year appropriations. The Government, as lessee, contemplates fulfilling all its? lease obligations and using the equipment described in the objectives (?Equipment?) for the full Lease Term. These terms and conditions shall be deemed incorporated by reference into any subsequent or renewal order for the Equipment during the Lease Term. The Government represents that the Equipment is essential to the daily operations of the Government mission supported by the Equipment. Upon request, the Government shall provide the Contractor with information to establish and document the essential use of the Equipment. Such Information shall include, but is not limited to, a description of the applications supported by the Equipment, and the planned life-cycle for the Equipment. All state and local taxes are excluded from the lease payments set forth herein. The Government agrees to pay or prove exemption from all taxes on the sale, use, or value of the Equipment lease under this schedule in the event the Government claims an exemption, but a state or local jurisdiction assesses such taxes against Contractor, or its Assignee. Government agrees to pay such taxes upon presentation of Contractors invoice. After payment, the Government may seek exemption under the State or local tax protest procedures; 5) Termination; 6) Order of precedence; 7) the terms and conditions in the Leasing Schedule control the lease and supersede any conflicting terms and conditions contained in the objectives; 8) Return of equipment; 8) Upgrades and additions; 9) Government furnished property and services; 10) The Government will provide the space and the power source (electricity) needed to complete this statement of objectives; 11) Invoice and payments; 12) Insurance Requirements; 13) Quality Control; 14) Performance Standards; 15) Available Funding: Funding is available for the first year obligation. Future funding will depend upon availability of appropriations for each subsequent year; 16) other proposed lease terms of the quoter.
