Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F1B3H16108A200 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-09 19 Apr 06. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 115112, with annual gross revenue not to exceed $6 million. Statement of Work: 1.1 All work performed under this contract is located at Altus AFB, Ok. 1.2 Work Hours: Regular work hours shall be Monday through Friday 0730 to 1630 (unless otherwise specified) and except for federal holidays. 1.3 Environmental Conditions: The contractor will not apply pesticides during high or gusty winds, during heavy rainfall or any other time when conditions are not favorable. 2. Government Furnished Utilities: The government will furnish a mixing location with access to water, when applicators need to refill tanks to complete that day?s work. 3. Contractor Furnished Items: The contractor shall furnish all equipment, materials, parts, supplies, chemicals and tools necessary to perform all work unless otherwise specified. 3.1 Certifications: All personnel involved with the pesticide applications will be certified in accordance with Oklahoma laws. All work shall be performed by a company performing pest control operations and licensed in the state of Oklahoma. Certifications and business license will be submitted to the Contracting Officer prior to starting contract. 3.2 Items of equipment to include safety gear (gloves, goggles, etc) necessary to perform work as required or ordered under this contract shall be furnished and maintained by the contractor. 3.3 Materials and supplies procured by the contractor shall comply with all state, local, and federal requirements. The contractor will submit copies of pesticide labels and material safety data sheets (MSDS), to contracting and entomology prior to starting the contract. The contractor shall submit on request, or the contracting officer may draw at anytime without prior notice, from stocks at the job site, samples of the insecticide used in the work. Should analysis indicate that samples contain less than proper amount of active ingredient specified on the label, all work performed with such product shall be repeated at no additional cost to the government. 3.4 The control of ornamental pest shall be accomplished with an Environmental Protection Agency approved non-corrosive pesticide. Use of any pesticide inconsistent with its label is a violation of federal law carrying severe penalties. Formulations and applications of pesticide will be in accordance with the approved label. Pesticide spills shall be immediately cleaned up by the contractor at no additional cost to the government. 3.5 Pesticides shall be delivered to the job site in the original labeled container. Labels shall bear the manufactures warning to be observed in handling and use of the material. 3.6 Pesticide dispersal equipment shall be inspected by the contractor prior to and during spray operations. Equipment shall be capable of accomplishing total treatment of all trees. 3.7 Pesticide usage reports containing: location(s) treated, chemical used and amount of active ingredient used will be given to entomology at the end of each work week. 4. Treatment Notification: 4.1 Pre-treatment notification will be accomplished in writing by 97 CES entomology section. Occupants will be advised on steps and precautions to take before treatment. 4.2 Post-treatment notification will be accomplished in writing and delivered to the occupants by the contractor. Supplemental Requirements: The work covered under this SOW is for spraying of infested elm and cedar trees in the housing area. Should all of the infected elms/cedar be covered and funds remain, the contractor shall extend the spraying operations to include sycamore and cypress trees. Spraying shall continue until all infected trees are covered or until funds are depleted; whichever occurs first. (End SOW). All work shall be performed IAW SOW. Award shall be made only to contractors who have registered with Central Contractor.
