Description

The 347th Contracting Squadron/MSCB is seeking sources to establish a Blanket Purchase Agreement (BPA) for a Moody Services 12 hour toll-free telephone support line. The objective of this BPA is to create an avenue for counseling 347 Medical Group (347 MDG) smoking cessation clientele in a cost effective manner. A telephonic service is desirable because such an arrangement improves client access, client success and cost. Clients should be able to access this service at no cost from any telephone within the Continental United States with 100% reliability during the hours of operation prescribed as follows. This service will offer access to clients Monday through Friday (except National Holidays) from 7:00 a.m. until 7:00 p.m. Eastern time. Trained, credentialed nicotine dependence counselors will utilize ?Freedom from Smoking? (evidenced based) protocols. Proposals should include contact information for a sample of current and/or historical customers. A Statement of Work for this requirement is listed as an attachment to this sources sought. Please indicate your interest as a potential source for this BPA by providing a quote for unit services no later than 22 May 2006, 2 PM Eastern. Your offer must include the name, address, and phone number of a point of contact. CCR enrollment is required for establishment of a Contract; registration can be initiated at www.ccr.gov.
