Description
It is anticipated that a firm-fixed priced, competitive purchase order will be issued for the procurement of 5020 total (20 First Article, 5000 Production) unit pack assemblies per the requirements and specifications herein. Requirements: CLIN 0001- First Article (FA)- The contractor shall manufacture twenty (20) FA samples per the referenced specifications and deliver to the Government for inspection and testing within 2 weeks of purchase order award. The Government will conduct a first a rticle test in accordance with ASTM D4169, Distribution Cycle 18, assurance level 1, acceptance criteria 1. The Government will provide approval for production within 1 week of delivery of FA samples. Upon receiving written approval to proceed with produ ction from the Government, the contractor shall commence with full production of the unit pack assemblies in the quantities identified herein. CLIN 0002- Production- Upon successful completion of the FA sample, the contractor shall manufacture a quantity of five thousand (5000) unit pack assemblies per the referenced specifications and deliver them to the Government. The contractor shall deliver 1000 of the production units, FOB destination, no later than 3 weeks after receipt of written notification of Government authorization of production and the balance of 4000 units within 5 weeks after receipt of written notification of Government authoriza tion of production. Specifications: Each unit pack assembly shall consist of the following parts: 1. the Top Pad shall be produced in accordance with Special Packaging Instruction (SPI) P5-15-31506 (attachment 2) and ASTM D 4727(attachment 1). 2. the Bottom Pad shall be produced in accordance with SPI P5-15-31506 and Mil-P-19644 (attachment3). 3. the Unit Pack Container shall be produced in accordance with SPI P5-15-31506 and ASTM D 5118 (attachment 4) , and the closure of the Container shall be conducted in accordance with ASTM D 1974 Method 2B6 (attachment 5). Top flaps of the boxes shall not be sealed by the contractor. Note: The Capped Conical Tube on SPI P5-15-31506 is for reference only and is not a requirement to be produced as part of this purchase order. The referenced specifications will be made available by request to the Contract Specialist. Inspection and Acceptance: Inspection shall be made in accordance with SPI P5-15-31506 Part II-Quality Provisions and Part III-Inspection Requirements Classification of Characteristics. This is a combined synopsis/solicitation for commercial items prepared in accordance with Subpart 12.6 as supplemented with additional information included in this notice. The solicitation is issued as a Request for Quotation (RFQ), number W52P1J-06-T-000 8. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2 005-09, Effective 19 Apr 2006. Quotations submitted in response to this solicitation shall include: (1) supplier part number, supplier part name, and unit price, inclusive of shipping, conforming to the minimum requirement, as stated above; (2) verification of delivery of as follows: CLIN 0001, First Article Lot, Quantity 20, delivery 2 weeks after award CLIN 0002, Production Lot, Quantity 1000, delivery 3 weeks after receipt of written notification of Government authorization of production, and Quantity 4000, delivery 5 weeks after receipt of written notification of Government authorization of production (3) verification that the items shall be individually preserved, packaged, marked, bar-coded and shipped, FOB Destination, in accordance with best commercial practices and shipped to: 1 Rock Island Arsenal Attn: Nino Bonavito-DFU Kits Building 131 Rock Island IL 61299 This procurement is a 100% set aside for Small Businesses.
