- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 249-Q150-06. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2005-09. The NAICS code for this solicitation is 325414. The small business size standard is 500 employees. Furnish and deliver the following UVAR XTS Procedural Kits for use with the Therakos Photopheresis machine. The SCHEDULE OF PRICES FOR THE BASE YEAR AND TWO OPTION YEARS shall be listed and priced as follows: CLIN 1. UVAR XTS Procedural Kit, XT125, 4 kits/case, est. quantity 90 cases/year, $______ per kit, $________Total. CLIN 2. UVAR Kits, XTS Procedural Kit, XT001, 4 kits/case, est. quantity 72 cases/year, $_______ per kit, $__________Total. Kits will be ordered by the VA on an as needed basis. Contract period will be for one year with 2 one-year options. Deliver FOB Destination to Medical Service, Nephrology Section, VA Medical Center, 1310 24th Ave., S., Nashville, TN 37212.
