- The Minnesota Army National Guard, through the United States Property and Fiscal Office for Minnesota, has a requirement for On-Site Medical Surveillance Testing. The actual testing will be required at three locations: Camp Ripley, Little Falls, MN; the Army Aviation Support Facility (AASF) at Holman Field, St. Paul, MN; and the 55th Civil Support Team/Weapons of Mass Destruction at the 33rd Airlift Wing, Air National Guard Base, St. Paul, MN. The following types of tests/surveillance are anticipated: Contractor set-Up Charge, Daily Fuel Charge, Work and Medical History Forms, Vision Acuity, Audiogram Exam, CBC W/Diff, UA, CBC W/Diff Blood PB & ZPP, CBC W/Diff, Cad Exposure Panel, Prostate Specific Antigen, Chest X Ray, Pulmonary Function Profile, Phys ical Exam, Make-Up (Estimate), Physical/Audiologist Referrals, Physical Exam, Head to Toe, Intra-ocular Pressure, Pelvic Exam, Rectal Exam w/stool for blood, Prostate, Chem-24 or equivalent, FBS (as in CH-24), CBC, Serum Cholinesterase (2 values), CPK, Ser um Lead, Serum Mercury, Hep C Sabay, Hep B Saby, PSA, CRP-cardiac, HIV, Lipid Profile (in C-24), Cholest (in C-24), HGB or HCT (in CBC), PAP Smear, Spirometry, Hearing, Vision, EKG, Random Urine Heavy Metal, Urine Micro, UA Drip, Nuclear Stress Test, CXR, Mammogram, CVSP, Record Review. The estimated solicitation issue date is o/a 1 May 2006. The estimated proposal due date is o/a 15 May 2006. A firm fixed-price Indefinite Delivery/Indefinite Quantity Contract is anticipated with a Base Year and four Opt ion Years. Evaluation criteria are past performance, capability, technical management and price proposal. This requirement is being reserved with preference for Small Business Set-Aside under a new and innovative concept. This concept is referred to as the Cascading Set-Aside for Small Business Awards . Large business concerns are put on notice of the preference for award to small business concerns. If two or more offers from qualified small business concerns are received, this solicitation shall be c onverted to a 100% Small Business Set-Aside, as defined by FAR Clause 52.219-6. In such event, FAR Clause 52.219-6, Notice of Total Small Business Set-Aside shall be considered incorporated into the solicitation by reference.
