Description

The following changes are to be incorporated in bold print in the revised Scope of Work: 1)deletion of the heaviest box(item # 16), thus requiring only 15 boxes to be shipped; 2)delivery to Elmendorf AFB, AK, changed to Friday, February 17, 2006, by 3:00 PM; and 3)proposals must be received by 8:00 AM, Honolulu Standard Time, February 16, 2006. See revised Scope of Work: REVISED SCOPE OF WORK FOR TRANSPORT OF CONTAINERIZED TACTICAL OPERATING SYSTEM FOR THE U.S. ARMY CORPS OF ENGINEERS, HONOLULU ENGINEER DISTRICT, FORT SHAFTER, HAWAII 15 February 2006. 1. SCOPE OF WORK: The Contractor shall furnish necessary management, labor, supervision, materials, equipment, transportation, and other incidental services to pick-up, load, and unload fifteen (15) Containerized Tactical Operating Center (CTOC) from the U.S. Army Engineer District, Honolulu, Bldg 525, 2nd Floor, Storage Room and transport via air to the U.S. Army Engineer District, Alaska, ATTN CEPOA-EM (D. Spence) 5223 Finletter, Stockroom, Elmendorf AFB, Alaska for delivery by 17 February 2006 between t he hours of 9:00 a.m.  3:00 p.m. Pick-up at Elmendorf AFB, deliver and unload at air freight terminal sixteen (16) CTOC from the U.S. Army Engineer District, Alaska, 5223 Finletter, Elmendorf AFB, Alaska on 24 February 2006 between the hours of 9:00a.m.  3:00 p.m. The 15 boxes must reach point of origin at Fort Shafter by 3 March, 4:00 PM. a. The Contractor shall transport equipment contained in a plastic/metal container, approximate total weight is 1,436 lbs. The Contractor shall provide an on-site representative to coordinate and direct all moves and serve as the point of contact with the Government. If a subcontractor is used during any part of the trip, he will report to the Honolulu on-site representat ive. The Contractors employees shall be under the supervision and direction of the Contractor. c. The Contractor is required to submit an Accident Prevention Program Administration Plan (POD Form 248-R (Rev) 1 Mar 95 and an Activity Hazard Analysis (POD Form 184-R (Rev) 22 Apr 91) to the Government Representative for review and acceptance within two (2) days following award. The Contractor shall not perform work until the Accident Prevention Program Administration Plan and Activity Hazard Analysis have been reviewed and accepted by the Government. The U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, which describes the required safety plans and forms may be found at the following web site: http://www.usace.army.mil/inet/usace-docs/eng-manuals/em385-1-1/toc.htm. P lease note that when a medical facility or physician is not accessible within five (5) minutes after an injury to a group of two (2) or more employees for the treatment of injuries, at least two (2) employees must be first aid and cardiopulmonary resuscita tion (CPR) certified by American Red Cross, its equivalent, or a licensed physician. The certificate shall not be older than three (3) years from date of issue, unless its certification period is specified otherwise by the issuing agency. d. The boxes are stored on metal shelves at the Fort Shafter location. Space in the storage room is very tight. e. The Contractor will -- (1) use the hallways as staging areas. The Contractor will ensure that there is clearance for people to walk down the hallways. (2) ensure that any items inc luded in the move will be staged in the hallway prior to it being moved. 2.
