- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-06-Q-0171 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-07. (iv) This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code 238220 and small business size standard $14,000,000. (v) The contractor shall provide the following items: supplies, tools, equipment, labor, personnel, supervision, and management, necessary to clean 1 chilled water system with 34 fan coil units and exhaust systems. For more information please see attached Statement of Work. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. STATEMENT OF WORK Performance Work Statement For Cleaning Air System at Finley Child Development Center 1.0. Description of services. The contractor shall provide supplies, tools, equipment, labor, personnel, supervision, and management, necessary to clean 1 chilled water system with 34 fan coil units and exhaust systems. Classrooms are connected to the exhaust system and well as having return grilles. Lockout/tagout the energy source as required by OSHA. Remove, clean, and replace ceiling diffusers, interior ductwork, and fan coil units for Finley Child Development Center, Davis-Monthan AFB, AZ. 1.1. Specific Tasks. All HVAC systems shall be cleaned in compliance with the latest edition of the following standards: 1. ACR 2002 Assessment, Cleaning, & Restoration of HVAC Systems (An industry standard developed by the National Air Duct Cleaners Associations, 2001) 2. Cleaning Fibrous Glass insulated Air Duct Ayatems, NAIMA (North American Insuation Manufacturers Association 1993). 3. HVAC Duct Construction Standard Metal and Flexible SMACNA (Sheet Metal and Air Conditioning National Association). 4. Standard 90-A, NFPA (National Fire Protection Association). 5. Standard 62-1989 and 55-1981, ASHRAE (American Society of Heating, Refrigeration and Air Conditioning Engineers). Contractor must be certified by NADCA (National Air Duct Cleaners Association) and have the following certified, educated, and experienced employees: 1. Certified Ventilation System Inspectors (CVI) 2. Air System Cleaning Specialists (ASCS) 3. Ventilation System Mold Remediator (VSMR) 2.0. Security Requirements 2.1. Listing Of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee's status or information changes. a. All contractor personnel will conduct themselves in a manner appropriate with the interests of good order and discipline of the installation. Any misconduct by contract personnel will not be tolerated. Minor work related disciplinary issues/violations may be delegated to the contractor for disposition by the contractor; however, if major problems occur such as pilferage/stealing, personal conduct, or other misconduct, the individual may be debarred from the base, lose driving privileges, be detained, or have other sanctions placed against them in accordance with DoD Directive 5200.8, ?Security of DoD Installations and Resources?. b. All contractor personnel will limit their travel on the installation only to specific areas required for performance of the contract, specified break and meal areas, or in travel directly to and from these locations. Employees found on the installation away from officially identified areas may be detained and/or debarred from the installation in accordance with DoD Directive 5200.8, ?Security of DoD Installations and Resources?. c. The government will conduct any background checks it deems appropriate and reasonable to maintain law and order and ensure the safety and security of the personnel and property on the installation. Installation access is contingent upon favorable completion of these checks. 2.2. Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles. 2.3. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc. 2.4. Traffic Laws. The contractor and its employees shall comply with all base traffic regulations. Failure to adhere to these regulations may result in loss of installation driving privileges and debarment from the installation. 2.5. Cell Phones. The use of other than hands free cell phones is strictly prohibited while driving on Davis-Monthan AFB. 2.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on (Name of Installation). 2.7. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment. a. The contractor shall not employ persons for work on this contract if such employees are identified to the contractor by the government as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population in accordance with DoD Directive 5200.8, ?Security of DoD Installations and Resources?. 2.8. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured. 2.9. Controlled/Restricted Areas. (IF APPLICABLE) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check (NAC), investigation before receiving a Restricted Area Badge. Interim access can be granted IAW AFI 31-501, Personnel Security Program Management. 2.10. Key Control. (IF APPLICABLE) The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government. a. The contractor shall immediately report to the Quality Assurance Evaluator (QAE) or Program Manager any occurrences of lost or duplicated keys. b. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor. c. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor?s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees
