Description

The VA Black Hills Health Care System with facilities located in Hot Springs and Fort Meade, SD intends to award a contract for a Community Based Outpatient Clinic (CBOC) which will provicde Primary Care services on an outpatient basis for approximately 516 beneficiaires with an estimated 1232 clinical visits per year for beneficaries living in the Winner, SD area. Area of service include the counties of Tripp, Todd and parts of Jones, Lyman, Mellette, Brule, Gregory, SD. Parts of counties in Nebraska are Cherry, Brown, Rock and Keya Paha. The CBOC is to be located within the city limits of Winner, SD. The CBOC shall be responsible for managing the veterans' primary medical care. Referrals for sub-specialty care and hospitalization shall be made to the VA Black Hills Health Care System. It is expected that the contract will be awarded shortly after February 10, 2006. However commencement of contract shall not start until June 4, 2006. Contract shall include a base one year contract period, with four additional option years. A Pre-Bid conference shall be scheduled with all potential bidders to answer questions and concerns. Offers are being solicted on a capitated basis. Offers must be received by close of business February 10, 2006, 4:30pm. Requests for the solicitation package must be in writing addressed to the attention of the Contracting Officer, (FIN 20), 500 North 5th Street, Hot Springs, SD 57747. All responsible offerors may submit an offer which shall be considered. Award of this solicitation is subject to the availability of funds. Applicable NAICS code is 621498, All Other Outpatient Care Centers.
