- This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. Solicitation RFQ 548-06-419. This solicitation is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-06. The associated NAICS is # 621512 with a size standard of $11.5 million. The VA Medical Center located at 7305 N. Military Trail, West Palm Beach, Florida, has a requirement for a mobile PET/CT scanner, including technician. MUST BE PET/CT AND NOT PET TECHNOLOGY ONLY. Lutetium Oxyorthosilicate (LSO) crystal technology required. It is anticipated a firm fixed price contract will be awarded. The contract period shall be January 17, 2006 through April 7, 2006. Prospective contractor?s shall submit a written quote, providing pricing on a per day basis. The Contractor shall be required to furnish services during normal VA business hours (8:00 am - 4:30 pm), excluding (Federal) national holidays. National (Federal) holidays are: New Year?s Day, Martin Luther King Jr. Birthday, President?s Day, Memorial Day, 4th of July, Labor Day, Columbus Day, Veterans? Day, Thanksgiving Day, Christmas Day. When one of the holidays falls on Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a national holiday by U.S. Government Agencies. Services shall be provided in accordance with the following special requirements and Statement of Work: Under the authority of Public Law 104-262 and 38 U.S.C. 8153, the Contractor agrees to provide all equipment, personnel, supervision, and supplies necessary to provide mobile PET/CT scanner services in accordance with the Statement of Work, terms and conditions stated herein. Special Requirements: The services to be performed by the Contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. The services to be performed by the Contractor will be under the direction of the Chief, Imaging Service. Contractor shall furnish the services covered by this contract, one (1) day per week. Qualifications: Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall be subject to review by the VA Chief, Imaging Service or designee. Credentials: technologist(s) shall possess CNMT (Certified Nuclear Medicine Technologist) and/or RT (Radiologic Technologist) certification; (a copy of license shall be provided with quote). Contractor shall be compliant with relevant regulatory and accrediting standards with respect to general environmental safety including, but not limited to, Office of Safety and Health Administration (OSHA), National Fire Protection Association (NFPA). The Contractor shall assume full responsibility for the protection of the personnel furnishing services under this contract, in accordance with the personnel policy of the Contractor, such as providing workman's compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. The parties agree that such personnel shall NOT be considered VA employees for any purpose and shall be considered employees of the Contractor. INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE: Before commencing work under this contract, the contractor shall furnish a copy of their Indemnification and Medical Liability Insurance certificate, as required by VAAR Clause 852.237-7, to the VA Contracting Officer that the coverage required has been obtained and such policy shall state: 'THIS POLICY SHALL NOT BE CHANGED OR CANCELLED WITHOUT WRITTEN NOTICE TO THE VA." Said policy must bear an appropriate "loss payable clause' to the United States as its interest may appear. Such evidence of insurance shall not be waived. CONTRACTOR COMPLIANCE: Contractor represents and warrants that it is in compliance with Medicare, Medicaid, and any other related federal, state, statutes, regulations, rules, policies and that neither they nor their agents, officers, employees, assigns or successors in interest are excluded from participation in the Medicare, Medicaid, or related federal or state programs. Further, Contractor shall immediately notify the Contacting Officer of the exclusion from such programs of any of it's agents, officers, employees, assigns, or successors in interest. Such exclusion shall be grounds for immediate termination of the contract. To ensure that the individuals providing services under the contract have not engaged in fraud or abuse regarding Section 1128 and 1128A of the Social Security Act regarding federal health care programs, the Contractor is required to check the Health and Human Services-Office of Inspector General (HHS/OIG) List of Excluded Individuals/Entities on the OIG Website (www.hhs.gov/oig) for each person providing services under this contract. Further, the Contractor is required to certify in its proposal that all persons listed in the Contractor's proposal have been compared against the OIG list and are NOT listed. During the performance of this contract, the Contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities. CONTRACT PERFORMANCE MONITORING: At a minimum, the following will be reviewed: Invoices will be certified by the COTR prior to payment. This will include a cross check against Imaging Service?s log of approved examinations and verification of billed charges. A log will be maintained in the contract file relative to problems or complaints associated with this contract. This log will document that mutually agreeable resolution is reached regarding each issue in a timely manner. Contractor shall perform 14 to 16 whole body scans per day, when applicable. VA RESPONSIBILITIES: One time transportation cost from Contractor site to VAMC, West Palm Beach; any permits required; foundation at site; utilities and connections; transportation of patient from VAMC to mobile unit; Isotope (dose) to be provided by VAMC WPB; Interpretation to be provided by VAMC WPB. MEDICAL CARE PROVIDERS: The Contractor and medical care providers furnishing services under the contract will cooperate fully with the Department of Veterans Affairs and the Department of Justice in the defense of any allegations of negligence brought against the Department of Veterans Affairs or the United States relating to medical care rendered during the term of this contract. This obligation applies whether or not the contract medical care provider is still performing services to the Department of Veterans Affairs and shall survive the term of this contract. This includes, but is not limited to, providing information as requested to attorneys from the Department of Veterans Affairs and the Department of Justice as well as being available for deposition and trial. This applies to derivative claims brought by a veteran/patient, spouse, friend, relative or companion of such patient, alleging bodily injury and/or death or similar claim, suit, proceeding or cause of action. Statement of Work: Contractor shall furnish all personnel, equipment, materials, supplies, supervision and equipment necessary to provide PET/CT scanner services, in accordance with this Statement of Work, terms and conditions of this solicitation. Mobile PET/CT Scanner services shall be performed at VA Medical Center 7305 N. Military Trail West Palm Beach, Florida 33410. Mobile PET/CT scanner shall be outfitted with a ramp or lifting device for non-walking patients. Contractor shall provide assistance, if necessary, to patient during examination. CT must be ?multislice? technology and not less than 4 detector row technology; 8 or greater detector row technology preferred; "Average" patient whole body scan time (varies with patient weight) shall be less than 20 minutes; Guaranteed compatibility with GE Centricity PACS (guarantee Integrating the Healthcare Enterprises (IHE) year 3 or better conformity) (modality worklist); C2 software version. MUST be capable of display on both VAMC WPB standard grey scale PACS monitors and on VAMC WPB PACS color monitors supported by GE Centricity version 2; Images transferred to PACS immediately on completion of each scan via network connection; QUALITY CONTROL: All studies will be protocol by VA Radiologists prior to any examination being performed. Studies will be performed according to VAMC, West Palm Beach protocols. INVOICES: All payments will be made monthly in arrears, after receipt of a properly executed invoice. All invoices shall contain the following information: Contract and Purchase Order Number; Dates of service SCHEDULE OF SERVICES: Mobile PET/CT scanner with technician to perform services as described in the Statement of Work estimated 12 days @ $_______.
