Description

Furnish all labor, transportation, materials, tools, equipment and supervision required to provide home oxygen services to beneficiaries of the Veterans Affairs Medical Center, Augusta, Georgia. Contract period to be April 1, 2006 through March 31, 2007, with 4 one-year options to extend. This procurement is being conducted under FAR Part 12 and FAR Part 15. The solicitation will be issued as a Request for Proposal (RFP). A technical proposal will be required. Technical evaluation criteria consists of: 1. Past Performance and Demonstrated Capability; 2. Quality Assurance Plan; 3. Personnel Qualifications; 4. Cost/Pricing. The NAICS Code is 621610 and the size standard is $11.5 million. This procurement is 100% set aside for small business concerns. The anticipated issue date is December 19, 2005. Request for hard copy solicitation and/or inquiries must be in writing to the Contracting Officer and will be accepted by e-mail or by fax at 478-274-5581. Any amendments issued to this solicitation will be available at the URL address. Interested parties are advised that they are responsible for obtaining amendments.
