Description
This is follow-on procurement for approximately twice monthly Mobile Lithotripsy Services, equipment and tech - for one year beginning 1/1/2006 through 12/31/2006 with one (1) optional one-year provision. Services shall be provided 1 or 2 days per month depending on need, every other Wednesday afternoon ? treating one to four patients. The contractor will provide a trained, licensed, experienced technician to assist a VA physician in the lithotripsy process. As a minimum, a transportable STORZ Modulith or better system is required. The equipment will treat all stones anywhere in the urinary tract and will accommodate patients up to 400 pounds, be modular and treat patients in the operating room or VA provided suitable location. Based on historical data, 25-40 patients per year will be treated. Some days, the clinic will be canceled when there are no veterans waiting for services. Equipment shall be state of the art, portable, self-contained, and meet all requirements established by JCAHO. It shall have the ability to perform endourologic procedures before and after ESWL without moving the patient. Malpractice insurance in the amount of at least one million dollars per occurrence is required. The technician shall provide evidence of training, licensures and expertise. Lithotripsy services shall be provided by a mobile unit provided at and for VAMC Alexandria, 2495 Shreveport Highway, Pineville LA 71360. The procurement provides non-invasive therapy for kidney stone disease at VAMC Alexandria to alleviate the need to refer veterans to other medical centers for stone removal. This is a continuation of an existing contract and all processes are in place. The VAMC Chief of Urology monitors and coordinates the program. VA shall provide the physician and administer required anesthesia. NAICS code is 621999 with size standard of $8.5 million average sales over the past three years. Accepted price will be firm fixed global per-procedure prices. VA will coordinate each visit with the contractor. Services are currently provided every other week on Wednesday afternoon. The unit shall be prepared and in plae to perform the first procedure of the day at 1:00 PM and continue to perform procedures until all scheduled veterans are treated. VA will inform the contractor of the tentative schedule on Friday before the scheduled Wednesday afternoon visit. This is a commercial solicitation authorized by 38 USC 8153.
