Description: Provide one licensed pharmacist to monitor pharmaceutical practices in the State Veterans Nursing Home, 200 Veterans Drive, Hazard, Kentucky 41701. Medication chart review shall be conducted monthly. Reviews will be done in no more than three (3) eight (8) hour days per month by the 10th workday of each month. This is being set aside 100 percent to a small business. The Government intends to award a firm fixed price contract to the Offeror who provide the best value to the Government. Period of performance will be a base year with four one-year option periods. Solicitation will be available on November 29, 2005.
