Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Pacific Region and Southwest Region Safety Huber Needles: Shoe and Head Covers Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Shoe and Head Covers. Pacific and Southwest Regions are military integrated delivery networks (IDNs), comprised of Army, Navy and Air Force medical treatment centers in California, Hawaii, Guam, Japan and Korea are in the process of standardizing Shoe and Head Covers. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for Shoe and Head Covers, your company is eligible to participate in the standardization process. Please respond by Email to: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than November 10, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture and/or distribute shoe and head covers? 2.Are your products available through the Prime Vendor, Owens & Minor? 3.What is your DAPA Number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Under what standards is your product manufactured? 6.Do your shoe and head covers meet OSHA standards? 7.Are your products latex free? 8.What types of shoe and head covers does your company manufacture? Do these include shoe covers that go up to the knee? 9.What level of protection do your head and shoe covers offer? 10.What materials are your shoe and head covers made of? Are these materials low lint? 11.Are your products designed to resist tearing under normal use? 12.What sizes do your shoe and head covers come in? Do you have sizes for shoe sizes greater than 10? 13.Does your company offer a one-size fits all for head and shoe covers? Does this size cover the entire shoe up to the ankle? 14.Are your head covers designed to minimize microbial dispersal? 15.Are your head covers designed to cover the head, side hair, facial hair, sideburns & neckline? 16.Are your shoe covers made of non-slip material? 17.Are your shoe covers designed to cover most styles of shoes and up to the ankle? 18.Are your shoe covers leak proof? 19.What types of educational tools or materials do you have for this product line? 20.What kind of staff training does your company provide? 21.Does your company provide customer service 24/7? 22.Is there any history of backorders and/or recalls for this product group?
