Description

This is Amendment Number 1 for combined synopsis/solicitation number 236-06-Q-0001 from the Indian Health Service in Rockville, Md. for a Simplified Acquisition, is for the purchase of packageing services and mass mailing of letters from the Rockville, Md. area to locations around the United States. PURPOSE: The Indian Health Service is requesting quotes for the mass mailing service which includes the assembly of scholarship material, stuffing packet envelopes, labeling, mailing and tracking service. The mass mailing service is necessary to ensure that IHS Scholarship applications reach Indian students and schools for FY 2006 by the third week of November 2006. The successful contractor?s mailing service shall be completed two weeks (14 days is the period of performance) after receipt of all package material. This project is to be completed locally within the Rockville Maryland area. There is no special circumstances set aside for Women Minority Business Enterprise or Minority Business Enterprise. A technical proposal is not needed at this time for this project. Heyman Mailing Service received the award for approximately $68,000 in the previous year for similar work. SCOPE OF WORK: The mass mailing service will include the assembly/collation, stuffing, sealing, labeling, mailing (individual & bulk mailing) and mail tracking service of approximately 30,000 Scholarship application packets which weighs approximately 8.5 oz. Approximately 10,000 will be mailed 1st class and the remaining 20,000 by bulk mail. Offers submitted shall include ALL cost i.e., labor, supplies, postage/shipping, transportation, etc. necessary to perform the mass mailing service: 1. Assemble/Collating ? Scholarship packets will require assemble/collating of the following material into a 12? envelope package: a. Area Office Scholarship Coordinator?s cover letter - (a letter from each of 13 Area Coordinators) the vendor will receive 13 different individual pre-printed letters to place one letter that is appropriate to the particular area that it is being mailed to, IHS will provide b. IHS HQE Scholarship Coordinator?s cover letter will be pre-printed and sent to the vendor to be place in all of the packages, IHS will provide. c. Application Instruction Book ? 8 ?? x 11? with thickness of 1/8?- IHS will provide. d. Application for Participation ? (Purple bubble booklet) ? 4 pages (8 back to back)-IHS will provide. e. Return envelopes shall be blank, size 9 1/2? x 12? and folded ? Vendor will provide. 2. Labeling envelopes a. IHS will provide an Excel file of address document identifying the names and addresses of individual and organizations to receiving individual or bulk mailing packets. b. The Excel file of address document will also identify which individual/organizational recipient will require First or Fourth class mailing requirement. c. The Excel file of sheet will identify number of bundles packets required. d. The contractor shall ensure that all address labels correctly reflect the address provided in the Excel file of address document with the sending and return addresses. 3. All printed material will supplied and sent to the vendor at one time. Supplies delivered to the vendor will incur no cost to the vendor. 4. Postage and shipping shall be included in bid for contract. 5. All packages will be sent to both individual physical addresses and post office box addresses 6. Tracking system ? The contractor shall provide a tracking system to document mailing dates, addresses, delivery date, receipt information and utilize in verifying if returned packets were due to contractor address typing error or IHS incorrect addresses error. Each piece will be tracked on a clear and concise format that can be electronically sent via email to IHS. The tracking report is expected at the completion of the project. 7. All packages sent will have a return address of the respective cover sheet letter provided. REPORTING REQUIREMENTS: The contractor shall provide the following reports: DESCRIBE REPORT TITLE, CONTENTS OF REPORT AND FORMAT. The report should include the following categories Number of individual packets mailed Number of Organizational packets mailed Number of packets mailed to Area Coordinators Number of undelivered scholarship packets Number of undelivered scholarship packets due to labeling error verified through tracking system With this Amendment and aditional information the due date for submission of proposals will be Oct. 27, 2005, at 5:00 PM Eastern Time. Any further questions must be submitted by Oct. 20,2005, 5:00 PM to both Larry Pope and Yolanda Reyna in writing. The work shall begin in early Nov. 2005 and run for 2 weeks.
