Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region HOSIERY ANTI EMBOLISM: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Hosiery Anti Embolism, also known as anti-embolism stockings or TED hoses Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Hosiery Anti Embolism. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) on Hosiery Anti Embolism, your company is eligible to participate in the standardization process. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than October 17, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture and/or distribute graduated compression anti-embolism stockings in a full line of sizes and lengths? 2.Are your products available through the Prime Vendor, Owens & Minor? 3.What is your DAPA Number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Under what standards is your product manufactured? 6.Are your products latex free? 7.What types of anti-embolism stockings does your company manufacture? 8.What is the range of compression for your product? 9.Are your products washable? What is the recommended method of drying your product after washing? 10.Does your company recommend ambulating while wearing your product? 11.What sizes do your anti-embolism stockings come in? Are they color-coded by size? 12.Does your company provide a measuring device and size chart for your product? 13.Does your product come with patient instructions for donning your product in the package? If so, in what languages are the instructions available? 14.What types of educational tools or materials do you have for this product line? 15.What kind of staff training does your company provide? 16.Does your company provide customer service 24/7? 17. Is there any history of backorders and/or recalls for this product group?
