- Notice of Sole Source Acquisition. The VA Palo Alto Health Care System intends on executing a follow on contract with SSI Surgical Services, Inc., 5776 Hoffner Ave., Suite 200 Orlando, Fl 32822 on 1 October 2005 throu 30 September 2006, with four (4) one year options. The vendor will perform the following: A. Basic scope of practice and purpose of contract: 1. The Contractor shall furnish all labor, materials, supplies, instruments, equipment, video equipment, and qualified technicians to support VA Palo Alto Health Care System (VAPAHCS) Endoscopy Procedures. The Contractor shall furnish above mentioned support as necessary to support all procedures identified in Exhibit A. 2. The contractor will provide all trocars and instruments for laparoscopic surgery cases available for each procedure (see Exhibit A). The contractor will own the instruments and manage their upkeep including all cleaning, repair, sharpening, and replacement when necessary. The contractor will charge VAPAHCS on a fee per case basis. The cost of disposable trocars and instruments plus the cost of maintaining reusable instrument sets is more than the fee charged by the contractor, thus the savings to the VAPAHCS. The contractor provides a trained technician to assist preoperatively with room setup, intraoperatively with verbal technical assistance with instruments and video equipment, and post-operatively with breakdown of equipment 3. The contractor shall furnish a maximum of two (2) qualified Endoscope Specialist and/or Manager to Veterans Affairs Palo Alto Health Care System to perform the following, but not be limited to: a. On-site personnel to assist during regular business hours (7am-4pm) Monday-Friday daily during all laparoscopic, arthroscopic, endoscope sinus, and urology procedures performed b. Standardization of instrumentation trays used in laparoscopic, arthroscopic, endoscope sinus, and urology procedures c. Purchase, maintain, repair, and replace instrumentation and specialty equipment, including video components, for use in laparoscopic, arthroscopic, endoscopic sinus, and urology procedures d. Troubleshoot video equipment (monitor, camera, light source, printer, DVD player, etc.), light cords, rigid, semi-rigid, and flexible scopes, and all related components for all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures e. Disposable component (suction irrigator, clear seals), reposable component (clear seals, trocars, valves, etc.) and reusable component (cannulas, irrigation probes, etc.) management, shipping & handling, and maintenance for all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures. B. Description of the Work 1. The contractor will purchase, maintain, repair, replace, and oversee total capital, disposable, and video equipment for the use in all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures. a. The contractor will supply disposable suction irrigators and clear seals, reposable trocars and shear tips, and reusable cannulas, instrumentation, and sleeves for all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures on a fee per use basis. b. The contractor will supply pre-operative, intra-operative, and post-operative technical assistance to physicians, scrub personnel, as well as OR Nurses to coordinate cost effective measures in all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures. c. The contractor personnel will manage and oversee pre-operative sterilization and processing, intra-operative troubleshooting and maintenance, as well as post-operative cleaning and repair management of all video towers, components, and systems used for laparoscopic, arthroscopic, endoscopic sinus, and urology procedures. d. The contractor employees are trained through a five week intensive process where classroom and in-field training cover the following: 1). Anatomy, Physiology, and Medical Terminology 2). Surgical procedures and instrumentation (all specialty areas) 3). Cleaning, sterilization processes, and peel packing 4). Video systems management and troubleshooting 5). Operating room protocols, techniques, and standards e. The contractor employees are supplied continuing education and clinical training through a routine of OR Consistencies and practical modules where the following areas are covered: 1). Laparoscopy (Gastric Bypass, Lap Chole, Lap Hernia, etc.) 2). Arthroscopy (Knee, Shoulder, Wrist, Ankle, etc.) 3). Thoracoscopy (Pleural Effusion, Lobectomy, etc.) 4). Endoscopic Sinus (Rhinoplasty, Polypectomy, etc.) 5). Urology (TUR, Bladder Examination, etc.) 6). Video Systems 7). Sterilization Processing (Blue-wrapping, Peel Packing, Tapes, Indicators, Parameters, documentation, etc.) f. The contractor adheres to and follows all applicable O.S.H.A., Joint Commission, A.O.R.N., and HIPAA protocols, standards, and policies mandated for all healthcare personnel that function either directly or indirectly with patient care within the Operating Room environment. g. The contractor conducts Account Surveys to collect data measuring the success of services provided, which are made available twice a year, every year of the contract term. h. The contractor prepares and presents Cost Analysis documentation to OR Administration as well as A&MMS personnel twice a year, every year of the contract term. i. The contractor will maintain at its sole expense Comprehensive General Liability Insurance with minimum limits of $1,000,000.00 per incident and $3,000,000.00 aggregate for all personnel supplied to perform sole or partial duties listed within contract agreement for the entirety of the term listed C. Location where the work is to be performed or delivery to be made: Department of Veterans Affairs ? Palo Alto Health System, 3801 Miranda Avenue, Palo Alto, CA 94304 D. Period of performance: October 1, 2005 to September 30, 2006. 1. The contractor will supply disposable suction irrigators and clear seals, reposable trocars and shear tips, and reusable cannulas, instrumentation, and sleeves for all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures on a fee per use basis. 2. The contractor will not transfer, separate, or bill directly for shipping, installation of products, or labor offered under any service agreement. 3. The contractor will supply a maximum of two (2) trained personnel on-site Monday-Friday, 7am-5pm daily for all laparoscopic, arthroscopic, endoscopic sinus, and urology procedures. 4. The contractor personnel are to be ready and available to perform duties for each day, Monday-Friday, 7am-5pm with a response time once contacted not to exceed 45 minutes. F. Will the Government be furnishing the Contractor any item to be used during the performance of the work? 1. The contractor personnel will be provided with the following: a. Office space. b. Shelving Units for instrumentation. c. Cleaning area with brushes, pipe-cleaners, and syringes. d. Peel pack, blue-wrap, and tip protector materials. e. Autoclaves, Steris Processors, and Sterrad Processors. f. Tapes, Indicators, and other related materials. G. Inspection Requirements. 1. The contractor will supply a copy of all related personnel documentation showing compliance with all immunizations, competencies, and governing body policies, protocols, and practices mandated for all personnel that function within the Operating Room environment. 2. The contractor will supply a copy of each and every invoice generated after all laparoscopic, arthroscopic, endoscopic sinus, and urology procedure performed for evaluation and/or necessary signatures.
