- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Proposals must be in this office not later than 1700 hrs, Sep 23, 2005. Electronic proposals are accepted. STATEMENT OF WORK PM Grease Separators @ Buildings 144, 146, and Food Wagon (203) 1.0 PART I GENERAL 1.1 General: Contractor shall provide all labor, equipment, and materials necessary to pump and scrape three (3) each grease separators on a monthly basis at Building 144 (Skedaddle Inn), Building 146 (Bowling Alley), and the Food Wagon (located at Buildi ng 203 in the mission area) at Sierra Army Depot. 1.2 Requirements: 1.2.1 Normal Requirements: Grease separators at Buildings 144, 146, and the Food Wagon will be serviced on the first Monday of every month excluding federal holidays. Whenever a federal holiday occurs on Monday, the service will be performed the Tuesday f ollowing the holiday. Sizes: Bldg. 144, 61 Gallons. Bldg. 146, 43 Gallons. Food Wagon, 15 Gallons. 1.2.2 Special Events: There may be times when some grease separators may require additional cleaning on an as needs basis due to additional use. The contractor shall be given a minimum of one (1) week notice. In the event that at least one (1) week notic e cannot be given, the contractor will be given as much notice as reasonably possible. . The contractor shall provide special cleaning based on a monthly rate. 1.3 Applicable Documents: 1.3.1 SIAD Regulation 420-7 (Fire Prevention and Protection) 1.3.2 SIAD Regulation 385-1 (Safety) 1.3.3 All local, state and federal laws 1.4 Pre-Construction Meeting: Prior to the commencement of work, a meeting will be held with the contractor and the government representatives to discuss matters of mutual interest concerning the contract. General conditions, work schedules and phasing, security, safety, depot siren, operations during, lightning/thunderstorm, character of work, operations & storage areas, housekeeping and clean-up, days of work, equipment, fire prevention, smoking areas, accidents, local rules and regulations, delays, qua lity assurance, inspections, digging permits, hazardous waste, debris disposal and other matters pertinent to work accomplishments will be discussed at this time. 1.5 Point of Contact: The Point of Contract (POC) for this action will be Dick Rock, 530-827-4376, email: Richard.w.rock@sierra.army.mil who is responsible for verifying that the contractors services have been rendered. 1.6 Work Coordination (Scheduling and Rescheduling): All work shall be coordinated with the POC. Work shall be scheduled or rescheduled at least two (2) weeks prior to starting work. 1.7 Smoking Areas: Smoking will be permitted only at locations designated by the POC. 1.7.1 Safety: 1.7.2 Regulations: All installation safety regulations shall be followed. 1.7.3 Signage: Appropriate signs shall be provided by the contractor and placed to mark work areas. 1.7.4 Work Area Closures: The work areas shall be closed off to unauthorized personnel. 1.7.5 Accidents: All accidents shall be reported to the Safety Office through the COR. This is to be accomplished within the same shift, preferably as soon after the accident as is practical. An occupational Safety and Health Administration (OSHA) Form 10 1 shall be submitted to the Safety Office within three working days after an accident. In the event of serious injury or damage, work will stop and everything will be left in place exactly as it was at the time of the accident, excepting when the possibili ty of more damage or injury exists. 1.8 Housekeeping and Cleanup: After completion of each phase of the work, the contractor shall leave all work areas in a neat, clean and workmanlike condition satisfactory to the Contracting Officers Representative (COR). After completion of the entire job, the contractor shall remove from the premises all tools, equipment, and materials. 1.9 Unscheduled Interruptions: Work operations are subject to unscheduled interruptions and contractor personnel may be required to immediately leave the designated work area or be denied access due to security operations. Mission requirements may necessi tate suspension or cancellation of custodial operations for that period. All incomplete work shall be rescheduled for completion at a later time/date mutually agreed upon by the QA personnel. 1.10 Work Verification: The grease separator duct sizes and other quantitative data stated in this contract have been computed from drawings or site measurements The contractor shall be responsible for the coordination and proper relationship of this work to the building premises, and thoroughly familiarize himself with all details of the work and working conditions, to verify all dimensions in the field, and advise the POC of any discrepancies before performing any work. 1.11 Quality Assurance: While performing this work the contractor shall be subject to on site inspections by the POC. If the work is found to be in violation of specification requirements, the Contracting Officer will issue a stop work order until the vi olation is resolved. Standby time and expenses required resolving the violation shall be the contractor expense. 1.12 Debris: 1.12.1 Non-Hazardous Waste: All scrap, solid waste, non-hazardous materials shall be disposed off the depot and in accordance with applicable laws and regulations at Contractor's expense, unless otherwise specified in the contract. 1.12.2 Hazardous Waste: Hazardous waste generated by Contractors on Sierra Army Depot shall be disposed of in accordance with Depot Policy, contact the Environmental Office, 530-827-4318, for proper disposal instructions. Government receptacles or contai ners shall not be used for disposal. 1.13 Delays: The government is not liable on any delay caused by the occupant, security inspections on the gate or any security measures imposed by the Installation Commander and/or higher authority. 1.14 Damages: Contractor shall be responsible for repairing damage caused by construction and or repair activities. Any damages incurred as a result of contractors actions shall be repaired or replaced at no additional cost to the government. 1.15 Materials and Equipment Stored on Depot: The government shall not assume any responsibility for the security or protection of any materials or equipment stored on the depot by the contractor. 1.16 Contract Additions and Deletions: The Government reserves the right to add or delete grease separators from the contract, subject to their change in use. Resulting contract price adjustments will be negotiated based upon costs identical per contract performance period. 1.17 CONTRACT PERFORMANCE PERIOD: Contract period of performance to include a base year covering twelve (12) months commencing approximately 1 October 2005 through 30 September 2006 with four (4) consecutive option years for a total contract period five ( 5) years. 2.0 PART II - PRODUCTS: 2.1 Contractor Supplied Materials: The contractor shall supply normal industry standard approved materials. 2.2 Government Supplied Materials: There are no government-supplied materials. 3.0 PART III - EXECUTION: 3.1 Cleaning: The contractor shall perform his work IAW normal industry practices and while performing the minimum requirements.
