Description
Request For Quotation Service Agreement For Television Repair & Installation RFP-598-110-06 Requests for Quotations are currently being accepted for service agreement Television Repair/Installations Service at John L McClellan Veterans Memorial Hospital, 4300 West 7th Street, Little Rock, Arkansas and Eugene Towbin Healthcare Center, 2200 Fort Roots Drive, North Little Rock, Arkansas for the period of October 1, 2005 through September 30, 2006 with two (2) one (1) year options.. Contractor shall provide all services necessary to maintain government-owned equipment (approximately 650-700 televisions) (Models RCA & Zenith & PDI) in operating order in accordance with manufacturer?s specifications. This proposed contract is set-aside for small business. These requirements and specifications are to include the complete servicing but not limited to the following to assure the equipment is maintained and performs in accordance to manufacturer specifications: 1. Labor to include repairs, parts replacement for unscheduled emergency and preventive maintenance. 2. Replace all worn, damaged, or spent parts with the original manufacturer?s replacement parts (new or warranted). Contractor must show evidence of access to original manufacturer?s parts. 3. Response time by telephone of four (4) hours, to work site sixteen (16) working hours for unscheduled or emergency service after notification of qualified technician and will proceed progressively to accomplish without undue delay. Contract representative will restore equipment to manufacturer?s specifications. 4. Contractor will sign in and out through Engineering Service, Bldg 1, North Little Rock, Arkansas and upon completion of service shall have the Contracting Officer?s Technical Representative. 5. A copy of the signed service report will be provided to the Contracting Officer?s Technical Representative. 6. Failure to provide required documentation for the record as required or failure to furnish service report as required may result in non-payment of charges. 7. The government reserves the right to have an Engineering Technician present during contract maintenance. This procurement is a total small business set aside. Successful bidder must be registered in the Central Contractor Registration Database and must provide a current certificate of liability insurance. All responsible sources may submit a quotation.
