- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3E35231A100 is being issued as a Request for Proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05 . (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 562991with a $6 million size standard. (v) Contractors shall submit monthly and annually pricing proposal for the 12-month base period and each year of the four 12-month option periods (see paragraph 1 and exhibit A of the attached statement of work). This proposal shall include cost for all labor and materials identified in the statement of work. All responsible sources may submit a quotation, which shall be considered by the agency. (vi) Contractor shall furnish and service portable toilets in accordance with the statement of work. (vii) Period of performance will be 1 Oct 05 ? 30 Sept 06 for the base period. 1st option period will be 1 Oct 06 ? 30 Sep 07, 2nd option period will be 1 Oct 07 ? 30 Sep 08, 3rd option period will be 1 Oct 08 ? 30 Sep 09, and the 4th option period will run 1 Oct 09 ? 30 Sep 2010. (viii - xii) The following clauses and provisions are incorporated and will remain in full force in any resultant award. The Government will award a contract resulting form this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the options. The following factors shall be used to evaluate offers: (1) Price and (2) Past Performance. The Government will evaluate offers for award purposes for the total price for the requirement. Technical and past performance, when combined, are equal. If the lowest priced evaluated responsible offer is judged to have an exceptional performance risk rating, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. Offerors shall provide 3-5 references of your most recent and relevant projects. (x) Contractors shall include a completed copy of FAR 52.212-3, Offerors Representation and Certifications Commercial Item. STATEMENT OF WORK 09 Aug 2005 Portable Toilets Rental for Charleston AFB, SC PRINCIPAL FEATURES OF WORK: 1. SCOPE OF WORK: Rental of portable toilet units under this contract will include delivery to the various sites, set-up for operation, periodic service, and subsequent removal from location to another. The contractor shall repair the respective sites to ensure that units are stabilized and readily accessible. Units shall be serviced two (2) times each week to include minor repair and parts. The contractor shall provide all labor and supplies necessary to secure the portable toilets in the event of a natural disaster (i.e. Hurricane). Units shall be placed on the site or moved from site locations within one (1) week, or sooner, upon notification by the Contracting Officer or his/her representative. The contractor shall provide all personnel, equipment, tools, supplies, materials, transportation and perform all operations necessary to supply, service and relocate portable toilets for Charleston AFB, SC. The contractor shall be required to respond within twenty-four (24) hours to an emergency service call. The location of the portable toilets shall be at various locations on Charleston AFB. All long-term use portable toilets are to be the heavy-duty type fiberglass to prevent being blown over. 2. Location of Work: The work shall be performed on location at Charleston AFB. 3. Government Property: The contractor shall be responsible for any property damage caused by him or his employees. 4. Clean Up: The contractor shall remove all debris caused by his work at the job site and at the end of each workday and dispose of it off base, at no additional cost to the government. Disposal shall be in strict accordance with local and State of South Carolina laws and ordinances. 5. Environmental Protection: The contractor shall comply with all local, State of South Carolina and Federal Environmental Protection Agency rules, regulations, laws and any other legal requirements for the protection of the environment. Materials or processes that use asbestos, beryllium or mercury shall not be used. 6. Utilities: All reasonable quantities of utilities will be made available to the Contractor without charge. Any temporary lines and connections that may be required will be installed, maintained and removed by the contractor at his expense and in a manner satisfactory to the Contracting Officer. Removal shall be accomplished prior to final acceptance. 7. Safety of Workers: The contractor and his employees shall comply with the Federal Occupational Safety and Health Standards and all rules, regulations, and orders issued pursuant to the "Occupational Safety and Health Act." 8. Pass and Identification: The contractor shall obtain the necessary passes and/or identification for entry into Charleston AFB for himself and all employees prior to commencement of work. The Government reserves the right to refuse to issue a pass to any employee of the contractor for any reason deemed valid by the Government.
