Description

This is a combined synopsis/solicitation issued for commercial items. The VA Network 22 Logistics Office, 5901 East Seventh Street, Long Beach, California 90822 intends to negotiate a commercial (FAR Part 12) sole source contract with Fresenius USA, Inc., 2637 Shadelands Drive, Walnut Creek, CA 94598 under the authority of FAR 6.302-1. A justification for other than full and open competition has been prepared in support of this acquisition. The Request for Proposal RFQ 600-340-05MH to purchase sixteen (16) " Fresenius 2008K System?, with Bicarb, SMS Option, Saline Drain Buckets, Internal Blood Pressure Module, 10cc Heparin Pump, Online Clearance, DiaSafe Plus and sixteen (16) Blood Temperature Modules is hereby issued for Commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2005-05. The NAICS Code is 334510 and the small business size standard is considered 500 employees. The systems are for the VA San Diego Healthcare System Nephrology Section, Dialysis Unit operates by providing a life supporting patient treatment with Fresenius equipment and products. THIS BOS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION ISSUED. A SEPARATE WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. Include in the price a one-year warranty, training, technical support, and installation. The equipment and supplies shall be delivered to the VA San Diego Healthcare System, West Side Loading Dock, 3350, La Jolla Village Drive, San Diego, CA 92161. Offeror shall submit a complete copy of provision of FAR 52.212-3, Offeror Representation and Certification-Commercial Items (Mar 2005). The following Federal Acquisition Regulation (FAR) Clauses and Provisions also apply to the acquisition: 52.212-1, Instruction to Offerors-Commercial Items (Jan 2005); 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2003); 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items (July 2005). In paragraph (b) of 52.212-5 the following apply: 52.203-6, 52.219-8, 52.222-3, 52.22-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-37, 52.225-1, 52.225-13, 52.225-15, and 52.232-34. In addition, incorporated herein by reference are the following: VA Acquisition Regulations (VAAR) Clauses and Provisions: 852.236-75, Guaranty (April 1984); 852.211-70, Requirements for Operating and Maintenance Manuals (Nov. 1984). Original proposal shall be submitted on company letterhead along with descriptive literature for all items offered to Michael I. Heilman, Contracting Officer (NBC/MP). A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
