Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the Simplified Acquisition procedures in FAR 13 and the format in subpart 12.6, as supplemented with additional information included in this notice. This annou ncement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. W911SA-05-T-0072 is issued as a Request for Quotation (RFQ) and is due by Sept 6, 2005. The RFQ is small business set-aside NAICS code 333 120. The Department of the Army, Directorate of Support Services is seeking business concerns to provide and install snow removal equipment on a 2004 Freightliner Model M2112 Tandem Axle Dump Truck with a 15 box. DESCRIPTION: CLIN 0001, purchase and insta ll with all controls and hoses 1 Hopper w/salt and sand spreader equal to a Henderson FSH II 15 304-Stainless Steel, top grate screens w/4 x 4 holes, heavy duty roller chain and shield. Must have front grease tubes extended to the rear. Front idler adju stments extended to the rear. Cost to include installation of a 20 steel spinner disc and UTE hold down kit. CLIN 0002, purchase and install with all controls, lights and hoses 1 power reversible snowplow 12 moldboard length / with heavy-duty frame with pin and loop hook-up, equal to BH-PNL 12-43. CLIN 0003 purchase and install with all hoses and controls 1 Mid-Mount Hydraulic wing blade 7 in length w/ light equal to an AHW/PDF II. CLIN 0004, purchase and install 1 complete kit equal to the Rud- Chain Ro togrip II automatic chain-grip system. Dump Truck will be delivered within 150 miles to the contractor for installation of equipment. The contractor is responsible for operation checks to ensure successful operation. Installation of equipment must be compl eted by November 5, 2005. The Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. PARTIES INTERESTED IN RESPONDING TO THIS RFQ may submit their quote in accordance with standard commerci al practice (i.e. quote form, letterhead, etc.) and MUST INCLUDE THE FOLLOWING INFORMATION: Solicitation number; (W911SA-05-T-0072) offerors complete mailing and remittance addresses; unit prices, extended total prices per CLIN, and grand total for all it ems; discount terms, DUNS number and Tax Identification Number. Prospective contractors shall complete electronic annual representations and certifications at http://orca.bpn.gov in conjunction with required registration in the Central Contractor Registra tion (CCR) database. Specifically, 52.212-3 Alt I shall be completed electronically at http://orca.bpn.gov by the solicitation due date to be eligible to receive an award for this solicitation. In accordance with FAR 52.212-1(k), prospective awardee shall be registered and active in the CCR database prior to award. If the Offeror does not become registered in the CCR database in the time prescribed by the Contracting Officer (NLT due date/time of solicitation), the Contracting Officer will proceed to awar d to the next otherwise successful registered Offeror.
