- Combination Synopsis/Solicitation Uniform Rental Solicitation Number: W9124R-05-T-0053 Response Date: Noon MST, August 31, 2005 Questions: Noon MST, August 24, 2005 This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Part 12 Acquisition of Commercial Items and Part 13 Simplified Acquisition Procedures, as supplemented with add itional information included in this notice. The solicitation document and incorporated provisions and clauses are those in effect through Fac 2005-05, Effective 27 July 2005 & Class Deviation 2005-o0001 and Defense Federal Acquisition Regulation Supplemen t (DFARs), current to DCN 200550726 Edition. This announcement constitutes the only solicitation. Request for Proposals (RFP) are being requested and a written solicitation will not be issued. The awarded contract will be firm fixed price. It is anticipate d that payment will be made by Government Visa Credit Card. The North American Industry Classification System (NAICS) is 812332 with a size standard of $12M. All prospective offerors must be actively registered in the Central Contractor Registration. Offer ors may register online at https://www.bpn.gov/CCR/scripts/index.html or http://www.ccr.gov The U.S. Army Yuma Proving Ground (USAYPG) has a requirement for uniform delivery and cleaning for multiple locations basewide. The contract will include a base period of one year plus (4) one-year option periods. Contractors shall propose a fixed price fo r the base year/4 option periods and the individual cost for the identified garments. The awarded contract will be firm fixed price. The proposal shall consist of Contract Line Item Number (CLIN) 0001, Quantity 1, Unit of Issue: Lot, Description: 6 employe es require uniforms, CLIN 0002, Quantity 1, Unit of Issue: Lot, Description: 25 employees require uniforms, CLIN 0003, Quantity 1, Unit of Issue: Lot, Description: 25 employees require uniforms, CLIN 0004, Quantity 1, Unit of Issue: Lot, Description: 5 emp loyees require uniforms, CLIN 0005, Quantity 1, Unit of Issue: Lot, Description: 14 employees require uniforms, CLIN 0006, Quantity 1, Unit of Issue: Lot, Description: Contractor Manpower requirements. Paragraph a. of provision 52.212-2 is completed as follows: i) past performance, and ii) price. Past performance is considered the mos t important evaluation for this acquisition; price is second in order of importance. Offerors shall include in their proposal, past performa nce information on at least three other contracts in which they provided the same or similar items.
