Description

The Carl T. Hayden VAMC, Phoenix, Arizona, is courier services from Community Based Outpatient Clinics (CBOC) of lab samples to the hospital laboratory at Carl T. Hayden VA Medical Center, Phoenix, AZ 85012-1892, for a base period of October 1, 2005 through September 30, 2006 with three option years and four possible three-month award-terms ending September 30, 2010, per the performance requirements in the solicitation 644-0002-06. The North American Industry Classification System Number (NAICS) for the services is 492110 and the Business Size Standard is 1,500 employees. The solicitation will be made available on or about August 19, 2005, with a due date of September 19, 2005, by 3:00PM Arizona MST. Copies of the solicitation may be obtained at no charge at the VA Business Opportunities System website http://www.bos.oamm.va.gov or the federal website at http://www.fedbizopps.gov/ Dissemination is limited to electronic medium and the VA will provide no hard copies. Any amendments issued to this solicitation will also be available at the URL address. This solicitation is a 100% set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) in accordance with FAR Subpart 19.14.
