- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announce constitutes the only solicitation; quotes are be ing requested and a written solicitation will not be issued. The solicitation number is W91YTV-05-T-0134. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect thr ough Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is 334510. This requirement is 100% total Small Business Set a-side. The Martin Army Community Hospital, Fort Benning, Ge orgia, has a requirement for the following laser equipment: CLIN 0001: Q-Clear 1064/532nm Q switch portable laser 1 Each $________ ; CLIN 0002: Q-Clear Stand compatible with laser 1 Each $__________; CLIN 0003: Ta2Eraser Tatto Removal Laser 1 Each $________; Total Price for all CLINs $___________. Please include shipping cost and date of shipment. The Government anticipates awarding a single award for this requirement. The items shall be delivered to the Martin Army Community Hospital, Bldg 9222 Santa Fe Road, Ft Benning, GA 31905-6006 within 30 days after receipt of order.
