Description: Contractor to fill first time prescriptions for VA patients seen at the Quincy Outpatient Clinic, 1707 North 12th Street, Quincy, IL, to provide medication counseling per OBRA 90 requirements to adhere to the VA formulatory, and to provide prescription pads for the writing of these prescriptions for the period of 10/1/05 (date of award) - 9/30/06 with two one year options included.
