Description
The Federal Bureau of Prisons, Community Corrections Contracting is seeking a Request for Quote from business concerns having the ability to provide outpatient substance abuse treatment and mental health services for federal offenders residing at the Community Corrections Center contract facility and those that are under home confinement in the Milwaukee, Wisconsin area. The treatment facility shall be located within ten (10) miles driving distance and accessible by public transportation to and from the contract facility located at 230 West Wells Street, Milwaukee, Wisconsin, 53205. Performance period and requirements under this contract will be: 1. Base period - 10/01/2005 - 09/30/2006: (a) Intake Assessment - 5 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 25 inmates) = 600 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 25 inmates) = 1,800 sessions,(d Mental Health Intake Assessments and Report = 4 each, (f) Medication Monitoring = 4 each; 2. Option Year 1 - 10/01/2006 - 09/30/2007:(a) Intake Assessment = 6 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 27 inmates) = 648 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 27 inmates) = 1,944 sessions, (d) Mental Health Intake Assessments and Report = 4 each, (e) Medication Monitoring = 4 each; 3. Option Year 2 - 10/01/2007 - 09/30/2008: (a) Intake Assessment = 6 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 27 inmates) = 648 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 27 inmates) = 1,944 sessions, (d) Mental Health Intake Assessments and Report = 4 each, (e) Medication Monitoring = 4 each; 4. Option Year 3 - 10/01/2008 - 09/30/2009: (a) Intake Assessment = 7 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 28 inmates) = 672 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 28 inmates) = 2,016 sessions, (d) Mental Health Intake Assessments and Report = 4 each, (f) Medication Monitoring = 4 each; 5. Option Year 4 - 10/01/2009 - 09/30/2010: (a) Intake Assessment = 8 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 30 inmates) = 720 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 30 inmates) = 2,160 sessions, (d) Mental Health Intake Assessments and Report = 4 each, (e) Medication Monitoring = 4 each. The services required above shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work dated January 2004. The solicitation will be available on or before July 8, 2005 and will be distributed solely through the General Services Administration?s website http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition including solicitation amendments will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about August 8, 2005. This is a full and open competition under NAICS code 621420 and the small business size standard at $8.5 million. Faith-based and Community-based organizations have the right to submit quotes equally with other organizations for contracts for which they are eligible. All responsible business concerns may submit a quote which will be considered by this agency.
