Description

This contract is for providing ostomy supplies to outpatients of the VA Salt Lake City Health Care System, Salt Lake City, Utah, and services to inpatients at time of discharge for the initial set up and instruction. Contractor shall maintain a stock of all items covered by this solicitation. A detailed item list will be provided as an attachment to the solicitation. Contractor shall provide a similar or equal item when stock item is not immediately available or in the case of an emergency. Substitution of any item shall require prior approval of the Enterostomal Therapy Nurse and the Chief of Pharmacy or designated representative and will be provided at no additional cost to the VASLCHCS unless prior approval of such cost is obtained. The government intends to enter into a Commercial Items contract, in accordance with FAR Part 12 and utilizing the negotiation procedures of FAR Part 15. Period of contract is for one base year and up to four option years. THIS SOLICITATION IS 100% SET ASIDE FOR SMALL BUSINESS.
