Description
Under the authority of 38 U.S.C. Section 8153, Federal Acquisition Regulations (FAR) Part 13 Simplified Acquisition Procedures and Veterans Affairs Acquisition Regulation (VAAR) Part 873. The Department of Veterans Affairs (VA) through the Consolidated Contracting Activity (CCA) located at the VA San Francisco Medical Center seeks a Contractor to provide Radiology Physician services. Contractor shall have the capacity to provide qualified radiology physicians to deliver a full range of MRI, CT, GI, general, chest & heart, and bone radiology services at the VA San Francisco Medical Center. Contractors Physicians services shall include but not be limited to the following: evaluation, triaging, sedation, previewing for exam quality, administering contrast, obtaining informed consent for contrast/procedures, and interpretation of the results. The services provided shall equal or exceed similar type of radiology physician services found in the commercial market place. The government has a need for approximately 2.4 Full Time Employee Equivalency (FTEE) and the government foresees an ongoing need at the same level for the foreseeable future. Therefore the government seeks a long term agreement (base-year plus four one-year option periods) with any potential Contractor. The need is currently being serviced via contract with a VA Affiliate and in the sprit of competition the contracting officer seeks all sources that are capable of providing the required commercial services. The contracting officer intends to follow the provisions outlined in VAAR 873.111 (d) Multiphase acquisition techniques which allow the government to initially seek limited information from potential vendors, make one or more down-selects and request a full proposal from an individual offeror or a limited number of offerors. In addition the contracting officer reserves the right to use the provisions of VAAR 873.111 (e) Alternative negotiation techniques. As provided in VAAR 873.111 (d) (2) this notice placed in the Government Point of Entry (GPE) VA Business Opportunity System (BOS) shall serve as the first phase notice to any potential offeror and as further outlined in 873.111 (d) (2) offerors are put on notice that failure to respond to this request for information shall make them ineligible to participate in further solicitation, award and contracts for the commercial services described herein. In accordance with VAAR 873.111 (d) (2) the government will make down-select decisions based on the response to the items listed below. a. Offeror shall provide a narrative statement outlining their qualifications. The narrative statement shall include but not be limited to the following: (i) The Offerors qualifications to provide full range of radiology physician services. (ii) Staffing, describe in detail how Contractor will staff this requirement, include a summary of the competency of service personnel, including credentials, licensure and a description on how they meet the requirements as outlined. Outline personnel recruitment strategies and ability to maintain sufficient qualified personnel to service the requirements. (iii) Provide a description of company organization, description of facilities, administrative structure, and list city or cities where facilities are located. b. Offeror must identify a Government or commercial customer for whom they have provided similar services in the recent past. Briefly describe the services provided to the customer, service periods, and approximate annual dollar value of the services provided.
