Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W91QF2-05-T-0010 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 05-02 dated March 23, 2005. This procurement is 100% Set-Aside for Small Business Concerns. The associated NAICS Code is 238990 with a Small Business Size Standard of $12.0. Contract line Item 0001. The project consists of painting and carpeting selected areas in Fort Hamiltons Building No. 114. Work shall be performed in accordance with the Governments Scope of Work. Description of Requirement. SCOPE OF WORK U.S. ARMY GARRISON FORT HAMILTON, DIRECTORATE OF PUBLIC WORKS-OPERATION AND SUPPORT DIVISION  129 WAINWRIGHT DRIVE, BROOKLYN NY 11252-6800 WORK REQUEST NO. IJO# AMD 00001-5J 1. SUBJECT: REPLACE CARPET & PAINT OFFICE AREAS, BUILDING 114 2. SCOPE: The work covered in this scope consists of furnishing all labor, expertise, equipment, materials, facilities, transportation, and appliances necessary to perform operations in connection with the work described in paragraph 3 SUMMARY OF WORK. Any perceived differences between this scope of work is to be resolved by the Contracting Officer or designated representative. 3. SUMMARY OF WORK: The contractor shall be responsible for providing all the technology, means, and methods in performing this work for the Government. Furthermore, the contractor shall fully adhere and comply with the approved Safety & Health and Enviro nmental plans, this projects specifications (where applicable), NFPA, and the NEC. The Contractor shall comply with all Occupational Safety and Health (OSHA), Uniform Federal Accessibility, and environmental regulations and standards. All code compliance and requirements questions are to be coordinated with DPW. The contractor shall perform the following work (but not limited to): BUILDING 114 - CARPETING THE CONTRACTOR WILL: - Remove & dispose of approximately 2300 S.F. of carpet [direct cement]. - Remove & dispose of approximately 650 L.F. of vinyl cove base. - Patch & prep. sub floor prior to installation of new carpet tiles. - Install approximately 300 S.Y. of carpet tiles[ As per supplied specs]. - Install approximately 650 L.F. of 4 vinyl cove base.[AMEDD POC 1Sgt. Dana Cochi will make color selection] Carpet tiles are to be installed in 11 different rooms. Contractor is responsible to move and reset furniture as needed. BUILDING 114 - PAINT OFFICE AREAS THE CONTRACTOR WILL: - Paint approximately 8500 S.F. of walls & ceiling [as per supplied drawing] 1 coat primer & 2 coats finish color. - Paint 32 doors and frames [as per supplied drawing] 1 coat primer & 2 coats finish color. - Patch / prep. walls & ceilings prior to painting. - Move & reset furniture as needed. - Install drop clothes to protect all adjacent areas. PAINT SELECTIONS: Office areas [walls] Mfr:Benjamin Moore Color: Bone White 03 Finish: Eggshell Trim-only:Semi-gloss Common Area-Restrooms Mfr: Benjamin Moore Color: Pacer White 6098 Finish: Eggshell Trim-only:Semi-gloss Office areas & common areas ceilings Mfr. Benjamin Moore Color: Brite White Finish: Flat 4. GENERAL NOTES: Point of contact and COR is Robert Downes or Drew Cavosi, who can be reached at (718) 630-4749 and (718) 630-4991 respectively. Contractor shall meet with the POC immediately to discuss problem areas, and shall be responsible for all w ork performed. Contractor shall provide a telephone number of person to call should the need arise. If any excavation is needed during this work the contractor is responsible to obtain a digging permit from the engineering chief Mr. Mike Paidoussis located in building 129. Contractor is responsible to schedule a performance period with the manager of the building, or person in charge and verify 1 week prior to start date .The Govt COR will also be notified. Any items that may have been unintentionally omitted from this specification, but which constitute acceptable practice in the trade, does not relieve the contractor of his responsibility to perform same. Contractor shall report to the Government COR, any dangerous or problematic condition which he may uncover in the course of executing this specification. On site inspection shall be performed prior to submittals, to verify measurements and field conditions. All work shall be performed by experienced workers with proper tools, equipment, and in a good workman like manner. Contractor shall remove all debris from the job site daily. Upon completion of the job, contractor shall remove all remaining debris from government property. Contractor shall leave the job site each day in a broom swept appearance and suitable cordoned off from the passage of non-construction personnel. Contractor shall register with the police at Bldg. 130 before commencement of work on a military post. Contractor shall supply COR with cuts of significant materials to be installed, name and quality for government approval prior to installation. 5. PERIOD OF PERFORMANCE: CARPETING: The period of performance shall proceed as follows. A pre-construction meeting will be held within 3 working days of award. Start of project will commence after scheduling with POC 1st Sgt. Dana Cochi , material availability & in coordination w ith a required painting project. The contractor will proceed the day after the painting contractor has completed 3 room. The contractor shall be able to install carpet in 3 rooms per day following painter. The completion of the project shall be within 5 wo rking days of award .The contractor will not be held responsible for any delays caused by the painting contractor. The bldg. POC will be required to have all computers moved. PAINTING: The period of performance shall proceed as follows. A preconstruction meeting will be held within 3 working days of award. Start of project will commence after scheduling with POC 1st Sgt. Dana Cochi , material availability & in coordination wit h the installation of carpet .The contractor will proceed after scheduling has been determined and is required to complete at least 3 rooms per day. The completion of the project shall be within 7 working days of award .The bldg. POC will be required to ha ve all computers moved. 6.ENVIRONMENTAL REQUIREMENTS: There are no specific Environmental concerns on this project. However it is the contractors responsibility to be aware for asbestos tile & insulation at all times. If you encounter any suspect material the contractor shall cease work and notify the government POCs identified above. Project shall be completed by 31 May 2005. The provision 52.212-1, Instructions to Offerors-Commercial Item, and 52.212.2, applies to this acquisition. Offerors are advised to include a completed copy of provision 52.212-3, Offeror Representations and Certifications- Commercial Items, with its offer.
