Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region Trocars, Disposable – Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Disposable Trocars. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Disposable Trocars. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for Disposable Trocars, your company is eligible to participate in the standardization process. If your company is not a current DAPA holder for this product line, you are still eligible to participate in the standardization process by contacting the Defense Supply Center, Philadelphia (DSCP), Mr. John Charalabidis at (215) 737-7124 or e-mail at john.charalabidis@dla.mil to apply for a DAPA. Your company can be considered once DAPA paperwork is pending at DSCP Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than April 26, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture and/or distribute a full line of disposable trocars? If so, please list the types of disposable trocars your company offers. 2.Are your products available through the Prime Vendor and/or Electronic Catalog (ECAT) supply system? 3.What is your DAPA or ECAT Number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Under what standards is your product manufactured? 6.Are your products latex free? 7.Are your trocars equipped with dilating or cutting obturators? 8.Do your trocars have a safety feature? If yes, what type? 9.Does the safety feature engage after penetration and remain engage after disposal? 10.Does your company manufacture and/or distribute disposable trocars in pediatric sizes? 11.Do you manufacture odd sized trocars and/or extra long for obese patients? 12.Do other manufacturers’ staplers and clip appliers fit through your trocars for the size specified? 13.Does your company make kits for the most common types of laparoscopic cases such as lap chole and lap nissen? If yes, what is included in the kit? 14.Are the plastic parts of your products recyclable? 15.Can your products be reprocessed? 16.What types of educational tools or materials do you have for this product line? 17.Does this product require staff training? If yes, is this training provided at no cost? 18.What does this staff training include? Are there CEU’s for this training? 19.Does your company offer assistance with the creation and maintenance of an inanimate laparoscopic training facility? If yes, please include the details and involvement required. 20.Does your company provide customer service 24/7? 21.Is there any history of backorders and/or recalls for this product group?
