- Description
- The U.S. Equal Employment Opportunity Commission (EE0C) seeks a contractor who can provide health services to include the necessary personnel, backup personnel, management, supervision, supplies, and medical equipment to conduct a comprehensive program of Occupational Health for the EEOC health unit. Approximately 600 employees will be served. The Health Unit shall be open Monday through Friday from 8:00 a.m. to 4:30 p.m., except Federal Government holidays. The contractor, at a minimum, shall provide one (1) full time staff nurse and one (1) part time staff physician (four hours per week) for the purpose of providing physicals and allergens, hormones, and other injections as needed, and requested. In the case of physical examinations, ten percent (10%) of the population will be authorized to received physicals per year. As to allergens, the physician shall be required to be at the health unit two (2) hours per week for that purpose. The contractor shall provide, at a minimum, fully qualified personnel for the required services. The contractor shall be responsible for purchasing and providing the supplies utilized in the health unit. The contractor shall maintain liability insurance coverage from a carrier acceptable to EEOC, for all personal injuries, property damage, and/or wrongful death arising from the performance of the contract. The contractor shall provide certificates evidencing said insurance coverage. The contractor shall provide the following treatments (a) occupational; (b) non-occupational; (c) physical examinations; (d) referral to private physician of employee with abnormal conditions requiring treatment by a private physician; (e) counseling; (f) screening and testing programs; (g) medical record keeping; (h) product assessment; and (i) supplies. A firm-fixed-price contract is contemplated. Duration of the contract estimated as a six month base period with four option years. The NAICS Code is 621111. EEOC anticipates issuing a solicitation in February 2005. The Closing Date for the solicitation will be thirty (30) calendar day after the issue date. The anticipated award date is April 1, 2005. Award will be made based on best value. All responsible sources may submit a bid, proposal, or quotation which shall be considered by EEOC. The solicitation will be made available to interested parties through electronic data interchange, FedBizOpps. This solicitation is a Small Business Set-aside.
