- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The acquisition procedures in FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). This solicitation number is 548-05-302. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. This acquisition is a single award and is unrestricted. A firm fixed price indefinite delivery, indefinite quantity order is anticipated. Contractor to furnish to the Department of Veterans Affairs Medical Center, 7305 N. Military Trail, West Palm Beach, Florida 33410-6400, the services and prices specified in the section titled, "Schedule of Supplies/Services" of this solicitation. The services specified in this contract may only be changed By written modification to this contract by VA Contracting Officer. Payment will be made in arrears upon submission of a properly prepared invoice. Invoices are to be numbered, fully itemized, dated with unit cost and total dollar amounts. All invoices shall be submitted to the following address: Financial Service Center P.O. Box 149971 Austin, Texas 78714 STATEMENT OF WORK: The purpose of this contract is to obtain a comprehensive agreement for the acquisition of whole blood, blood components and derivatives of human blood for the Pathology and Laboratory Medicine Service, Veteran's Affair Medical Center (VAMC), West Palm Beach, Florida. Services from this contract will supply all blood components used at the West Palm Beach VAMC for the time period February 12, 2005 through April 30, 2005. The contractor will provide human blood components and specialized procedures upon request of the VAMC, West Palm Beach, FL. The following services must be provided: ?Contractor agrees that its blood center(s) will furnish all personnel, supplies and facilities necessary to collect, process, store and deliver Human blood components and derivatives. All blood components and derivatives that are provided under this contract will be collected, processed, and stored in accordance with Food and Drug Administration (FDA) Regulations (21 CFR) and applicable contractor standard operating procedures under contractor's US License. Specifically, contractor agrees that all components shipped will have been tested and found to be non-reactive for HBsAg, RPR, NAT testing for HIV & HCV, HBcAb, antibodies to HIV-1, HIV-2, HCV, and HTLV I/II. Additional tests will be required as mandated by the FDA. ?Human blood components provided by contractor will include, but not be limited to, packed red blood cells, whole blood, platelet concentrate (single unit), platelet by apheresis, washed red blood cells, frozen red blood cells, leukocyte poor red blood cells, leukocyte concentrate by apheresis, fresh frozen plasma, cryo-poor plasma and cryoprecipitate. ? Specialized procedures provided by contractor will include, as a minimum, in-house flow cytometric analysis; in-house HLA services, including DNA analysis; and in-house apheresis procedures. Contractor agrees to provide specimen pick up and delivery services as requested by VAMC on a twenty-four (24) hour per day, seven days per week availability, including all Holidays. Contractor agrees to provide blood component delivery to the VAMC on a twenty-four (24) hour per day, seven days per week availability, including all Holidays; with routine delivery within 4 hours and emergency delivery within 1 hour. If the contractor fails to meet these delivery requirements, the contract may be terminated by the government due to failure to perform, and will include penalties as allowed. Contractor will ensure provision of blood and blood products for all emergent cases. The Contracting Officer?s Technical Representative (COTR) reserves the right to determine which cases are classified as emergent. Contractor will provide routine test results within 24-48 hours of specimen pick-up, unless specified in advance. STAT (emergency) test results shall be reported within 8 hours or less of specimen pick-up. Specimens and blood components must be transported in such a way as to ensure the integrity of specimens and blood components, and comply will accreditation standards as defined by the FDA and American Association of Blood Banks (AABB). Contractor shall provide all supplies necessary for shipment of blood components and specimens. Contractor will provide any special preservatives for specimen preservation. Frozen specimens and components will be shipped on dry ice provided by contractor. VAMC agrees to return all blood shipment boxes to contractor. Licensing and Accreditation: Contractor shall have all licenses, permits, accreditation and certificates required by local, state and federal laws. Contractor shall be accredited by the U.S. Food and Drug Administration (FDA), and the American Association of Blood Banks (AABB) and will submit proof of this accreditation with the bid. Accreditation by Centers for Prevention and Disease Control (CDC) is required if contractor is engaged in interstate commerce. Contractor's Medical Director is a licensed physician. Changes: Any proposed subcontractor changes from what is proposed under this contract must have prior approval by the VAMC Contracting Officer. Contractor shall be responsible for finding an outside laboratory with appropriate licensure and accreditation to perform tests that the contractor cannot perform. Contractor shall notify VAMC Laboratory and Contracting Officer of any change of subcontract. Any changes shall be submitted to the Contracting Officer for review and approval. Technical Administrative Liaison: In the interest of maintaining blood banking practices which are consistent with objectives of the Veteran's Affairs, the VAMC, West Palm Beach, FL, will : Appoint a Contracting Officer?s Technical Representative (COTR) to act as liaison with the Contractor Medical Director. Encourage the maximum clinical use of red blood cells with a minimal goal of ninety (90) percent. Participate with the contractor in an agreed upon blood inventory Control System. Whole blood and/or red blood cells committed for use which become 21 days old, collected in CPD anticoagulant, and not used, will be returned to the contractor, for credit, no later than 24 days after the collection date. Whole blood and/or red blood cells committed for use which become 35 days old, collected in CPD-A anticoagulant, and not used, will be returned to the contractor, for credit, no later than 38 days after the collection date. Whole blood and/or red blood cells committed for use which become 42 days old, collected in ADSOL anticoagulant, and not used, will be returned to the contractor, for credit, no later than 45 days after the collection date. Returns: VAMC, West Palm Beach, FL, may participate with other local facilities in the transfer and use of whole blood, red blood cells, and other components for their most efficient use. The VAMC will return to and receive credit from the contractor for acceptable units of whole blood or blood components originally provided by the contractor. Blood components provided by the contractor with greater than 9 months dating will not be returnable except in the case of defect or damage. Ordering: The amounts indicated are estimates of the quantities of blood and components that will be furnished during the contract period. These amounts may be increased or decreased without formal amendment to this agreement based on the actual needs of the VAMC and available supplies of blood and blood components in the contractor blood center. The VAMC will issue telephone orders for red blood cells and other blood components as needed. VAMC will specify group and type wherever applicable and indicate tentative delivery schedule. Emergency orders will be placed by telephone. Invoice will reference contract and purchase order number, indicate all testing procedures performed and the number of units of Whole Blood, Red Blood Cells, Blood Components delivered less the number of units of blood and blood components returned to the contractor and accepted for credit. Other Information: Award of this contract will not preclude the VAMC, West Palm Beach, Florida from acquiring blood or blood components from an alternate source if the contractor cannot supply blood or blood products for routine use. FOB Dest delivery to VA Medical Center, 7305 North Military Trail, West Palm Beach, FL 33410-6400. The provision at 52.212-1, Instructions to Offerors ? Commercial, applies to this acquisition. Addendum to FAR 52.212-1, Instructions to Offerors- Commercial Items ? Sub-Part 13.5 Test Program applies. FAR 52.212-2, Evaluation- Commercial Items applies. The evaluation criteria will be based upon price and the offeror must be able to meet the requirements listed above. The Government intends to evaluate the information received and award without discussion. Quotes shall be submitted and shall include the information required in FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items (Jan 2005). Contractor shall also provide sufficient evidence that have adequate equipment, facilities, personnel and necessary experience to establish responsibility to meet the above requirements by submitted three references of companies whom exact supplies/services have been furnished.
