Description

The Bureau of the Census is issuing this notice as a combined synopsis / solicitation for the purchase of three (3) each Phonetree 3500 telephone messaging system ? rack mount 24 port and PhoneTree annual support package. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. Solicitation 51-SO-BC-5-0006 is issued as a Request For Quote. This solicitation and incorporated provisions and clauses are those in effect through FAC 2001-25. CLIN001 PhoneTree 3500 telephone messaging system, rack mount. CLIN002 PhoneTree annual Support package for CLIN001. Delivery is FOB destination to the Bureau of the Census, 1201 East 10th Street, Jeffersonville, IN 47132, Attn: Receiving Section Bldg. 60 F. Acceptance at same location. Delivery on or before 01-25-05. Awardees will have to register at CCR.gov and complete a U.S. Census Bureau?s vender registration form. The following provisions and clauses apply to this RFQ. 52.212-1, Instructions to Offerors-Commercial Items, 52.212-2, Evalutions-Commerical Items, 52-212-3, Offeror Representations and Certifications-Commerical Items, include a completed copy with offer, 52.212-4, Contract Terms and Conditions-Commerical Items, 52.216-22, Brand Name or Equal (Aug. 1999) and 52-214-20, Bid Samples (Apr. 2002). Be sure to include all shipping and handling charges to the delivery location. Bids are due by 5:00PM on 28-Dec-2004.
