Description

MODIFICATION IS ISSUED TO ADD BUY INDIAN FIRMS TO THE SOCIO-ECONOMIC CATEGORIES. SOURCES SOUGHT ONLY ? The Department of Health and Human Service, Phoenix Area Indian Health Service, Phoenix, Arizona, is seeking CAMTS (Commission on Accreditations of Medical Transport Systems) certified firms from the following Socio-Economic categories, i.e., 8(a) Program Participant, HUB Zone, Service Disabled Veteran Owned, Small Business, Small Disadvantaged, Veteran Owned, and Women owned Businesses, to provide medical air ambulance services for eligible Native American beneficiaries of the Whiteriver PHS Indian Hospital, Whiteriver, Arizona; San Carlos PHS Indian Hospital, San Carlos, Arizona; Parker Indian Health Center, Parker, Arizona; and the Hopi Health Care Center, Polacca, Arizona, to various hospitals located in the State of Arizona, California, and New Mexico, for a period of one (1) year with four (4) option years. Solicitation packages are not available at this time. Estimated volume of transport is 13,000 projected for five years, from various locations listed above. Qualified firms in any of the above socio-economic categories are encouraged to submit statement of qualifications. Interested firms must have all the necessary equipment, personnel, experience, capacity and capability to perform the required services.
