Description

The Department of Veterans Affairs, Central Texas Veterans Health Care System has a requirement for a Capitated Community Based Outpatient Clinic (CBOC) for continuous primary and preventive medical care services and mental health services for VA enrolled veterans in Marlin, Texas. The Offeror(s) place of performance/facility to provide services must be located in Marlin, Texas. The intent is to provide primary and preventive medical services and mental health services by a local provider within the community. Services will be provided through scheduled visits. Service reimbursement rate shall be an all-inclusive capitated rate per enrolled patient per year. The contract effective period shall be from the date of award through September 30, 2005 with four (4) one-year optional renewal periods. The solicitation will be issued 11/4/04, with proposals due by 4:00 PM, Monday, 1/3/05. A pre-proposal conference is scheduled for Tuesday, 11/30/04, at 2:00 PM at the Central Texas Veterans Health Care System, 1901 Veterans Memorial Drive, Bldg 171, Rm A-25, Temple, TX 76504.
