- This is a combined synopsis/solicitation for commercial items prepared with the format in Subpart 12.6, as supplemented with additional information included in this notice This announcement constitutes the only solicitation; Request for Quotations (RFQ) are being requested and a written solicitation will not be issued. Solicitation documents incorporated provisions and clauses will not be issued. Solicitation documents incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-24. This procurement is unrestricted. The NAICS is 561790 with business size standard of $6 Million. A firm-fixed price contract is anticipated. The VA Puget Sound Healthcare System, 1660 South, Columbian Way, Seattle, WA 98108 has a requirement to contract for Ventilation Hood Cleaning Services at both the Seattle Campus, situated at 1660 Columbian Way, Seattle, WA, and the Tacoma Campus, situated at 9600 Veterans Drive, Tacoma, WA, to provide Ventilation/Hoods Cleaning to reduce the potential for grease fire for the American Lake and Seattle Divisions IAW NFPA 96. Commercial cleaning of Class One Kitchen Ventilation Exhaust System in B-100 Canteen (2-hoods cleaned quarterly), B-22 Burger King (5-hoods, 1 cleaned monthly at Burger King location and other 4 cleaned quarterly) for the Seattle Division. At the American Lake Division B-132 Canteen (1-hood cleaned 3 times per year), B-100 Basement Dietetics (7-hoods, cleaned 3 times per year), and B-3 Dietetics (2-hoods cleaned 3 times per year). Quotations shall be submitted F.O.B. Destination. All quotations are to include a completed copy of the provisions at 52.212-3, Offer Representations and Certifications-Commercial Items, with quotations, (a copy of this provision may be requested by facsimile to (253)589-4140, attn: Mike Gibbs, Contracting Officer). The following provisions and clauses apply to this action; 52.212-4 Contract Terms and Conditions Commercial Items, 52.212-5 Contract Terms and Conditions required to implement statutes and Executive Orders-Commercial Items including clauses incorporated by reference under Section b (11-16)(19)(21-22)(25), 52.212-1 Instructions to Offerors-Commercial Items, VAAR Clause 852.211-77 Brand Name or Equal, 852.211-70 Requirements for Operating and Maintenance Manuals, 852.211-71 Guarantee Clause and 852.270-4 Commercial Advertising.
