Description
This a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are h ereby requested. A written solicitation will not be issued. This is issued as a Request for Quote (RFQ) Number 04-10. (A) SPECIFICATIONS: The Utah National Guard has a need to have modifications made to five (5) Blazers for recruiting purposes. Modifications to include, but are limited to: (1) 6 suspension lift kit; (2) 35 chrome tires and rims; (3) new bench seats; (4 ) molded carpet for vehicle interior; (5) chromed stop rails (smurf bar); (6) chrome grill assembly; (7) rear axle to handle tires at road speeds; (8) 5 year warranty on the equipment and a lifetime warranty on the installation. Quote should include all p arts, labor, installation and any other related costs involved in lifting the vehicle. All parts to be high quality. Vehicles are available for inspection by contacting SFC Paul Raymond at 801-523-4428. (B) APPLICABLE PROVISIONS AND CLAUSES: (1) FAR 52.212-1, Instructions to Offeror--Commercial Items, Addendum paragraph (h) Multiple Awards - delete in its entirety, the government plans to award a single contract resulting from this solicitation; (2) FAR 52.212-3, Offeror Representations and Certifications--Commercial Items; (3) FAR 52.212-4, Contract Terms and Conditions--Commercial Items; (4) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes and Executive Orders--Commercial Items , the following clauses are applicable and incorporated by reference: FAR 52.203-6, Restrictions of Subcontractors; FAR 52.203-10, Price or Fee Adjustment for Illegal or Improper Activity; FAR 52.219-8 Utilization of Small Business; FAR 52.219-14 Limitatio ns on Subcontracting; FAR 52.222-21, Prohibition of Segregated Facilities; FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52. 222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era; FAR 52.232-33 Payment by Electronic Funds Transfer--Central Contractor Registration; (5) DFAR 252.212-7001, Contract Terms and Conditions Required to Implement Statute s or Executive Orders Applicable to Defense Acquisitions of Commercial Items; (C) EVALUATION FACTORS: The contract type for this procurement will be fixed price and award will be made on the basis of best value to the Government, price and other factors considered. Selection will be based on the following evaluation factors, in ord er of importance: (1) the offerors capability to provide the items required, and the quality of the proposed items; (2) past performance, caliber of offerors performance on previous work similar to work required under this RFQ; and (3) total cost or price . (D) INSTRUCTIONS: Offers shall include (1) a cost quote to include descriptive literature of products being proposed; (2) FAR 52.212-3 Representations and Certifications, Commercial Items, filled in as applicable (a copy of FAR 52.212-3 is available in ful l text at www.arnet.gov/far or upon request from this office; (3) a list of at least three references for whom you have done similar work. Provide the following information for each reference: (a) name and telephone number; (b) when the work was performed ; and (c) a brief description of the work. Failure to provide items 1-3 listed above with your offer, may cause your offer to be considered non-responsive. To receive award, the offeror must be registered in the Central Contractor Registration (CCR) data base, which can be accessed at www.ccr.gov. Offers shall be received NLT 2:00 P.M. MDT on September 20, 2004 to USPFO for Utah, Attn: Sherri Shepherd, 12953 South Minuteman Drive, Draper, UT 84020-2000 or by email at sherri.shepherd@ut.ngb.army.mi l. All offers must arrive at the place and by the time specified.
