- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is V15-04-0094. The solicitation is a Request For Proposals. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. This acquisition is unrestricted. The NAICS Code is 621111, and the size standard is 8.5 million. The requirements are for Radiation Therapy Consultation and Treatment, Brachytherapy (Prostate only, all other sites will be approved on a case-by-case basis) and Intensity Modulated Radiation Therapy (IMRT). The facilities serviced will be the VA Medical Center, Kansas City, MO; the Dwight D. Eisenhower VA Medical Center, Leavenworth, KS and the Colmery-O'Neil VA Medical Center, Topeka, KS. The provisions at 52.212-1, Instructions to Offerors-Commercial, apply to this acquisition, as does 52.212-2, Evaluation-Commercial Items. Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items also appllies to this acquisition, as does 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items.
