Description: (i) This is combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA5205-04-Q-S009. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 (as of 18 Jun 04). (iv) This procurement is a Total Small Business Set-aside acquisition. The NAICS code 811219 applies to this solicitation. (v) This RFQ contains one (1) line item. Unit of measure is lot. (vi) Line Item 0001: Non-Personal Service. The contractor shall furnish all labor, tools, equipments, and transportation necessary for maintenance of millennium access control system (*see below for detail) located in Bldg 96, 99 and 100 at Misawa AB, Japan. Maintenance includes cleaning readers, checking systems and standard fuses, battery fuse(s) back-up, data base back-up, maglock and/or hardware adjustments (not including any replacement parts if necessary), archive history and any additional training for on-site staff (how to use the system program). The maintenance will be twice a year, in Apr 2005 and Sep 2005. *Millennium Access Control Systems were manufactured by Kaba Ilco and installed by Vegas Valley in Bldg. No. 96, 99, and 100, 35th Medical group at Misawa AB, Japan on Feb 2002 and Nov 2003. Millennium access control system is PC based card access and controls when and where a person goes within a building or series of the buildings. The system uses a computer for card badge printing and encoding and adding or changing how the doors and users operate. (vii) Service will be performed at 35th Medical Group at Misawa AB, Japan and accepted by SrA Arcega, 35 MDSS/SGSLF. Award will be made to the responsible offeror whose quote/offer represents the best value in terms of technical (capability of item(s) offered to meet the government need), price, and past performance, which will be most advantageous to the government.
