Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Parts 12 and 13.5 using Simplified Acquisition Procedures (SAP). This announcement constitutes the only solicitation; quotations are being accepted and a written solicitation will not be issued. Because of the unique involvement with the Air Force programs and the development of the official Air Force Community Center Trademark Event Manual, the government intends to award a sole source requirement to, Wiersma Experience Marketing and Ms. Betsy Wiersma. Purchase Request Number F4SVP242310100 is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-14. The NAICS Code is 611430; size standard $6M. Commerce Business Daily Note 1 applies to this requirement. STATEMENT OF WORK for FAMILY MEMBER PROGRAM TRAINING SERVICES 1. Description of Services: The purpose of this Statement of Work (SOW) is for the Contractor to provide a training session October 2004 in conjunction with the AF Community Center training and the National Recreation and Park Association national conference. This training session will be for one 8-hour block for community center directors and recreation managers. This training event will include a high-energy presentation, review of the existing Air Force installation Trademark Events, interviews of community center directors? best practices and successful programs for multiple PowerPoint presentations, and will culminate in the presentation of the new chapter for the Community Center Trademark Event Manual, Contests, Tournaments, and Competitions. Wiersma Experience Marketing has developed this additional chapter for the AF Trademark Event manual. 2. Services Delivery Summary: The contractor will provide training topics including but not limited to the event/project planning, creativity exercises, new Trademark chapter ideas, group planning and hands-on involvement, and interviews of community center staff on best practices and successful events. Training will specifically address relevant programming processes to help the community center staff in their support of their communities. The Contractor will provide training and technical advice to community center directors, program directors, youth directors, and major command staff. 3. Performance Objective: The contractor will provide formal and small group training tailored to specific requirements/needs of the group. The contractor will mail or bring materials such as handouts to the training as necessary to support training topics. The success of this training will be determined from participants? evaluations/surveys at the end of training. 4. Travel: Reimbursement of travel and travel related expenses incurred in the performance of the Statement of Work shall be reimbursed for the expenses not to exceed the limits set forth in current Joint Travel Regulations (JTR Vol. 2, C6004) and Federal Acquisition Regulations (FAR 31.205-46). All travel to be performed by the contractor shall be pre-approved prior to any travel. Once approved, the contractor shall make arrangements and pay all necessary commercial travel expenses. After completion of travel, the contractor shall submit expense vouchers to HQ AFSVA/SVPY. 5. Special Qualifications: Consultant will always present a neat, professional appearance. 6. Government Inputs and Equipment: The government will furnish or make available to the contractor any documentation deemed necessary to accomplish this training. Also, the Government is prepared to provide the following documents and office materials: a. Data: The Government has unlimited rights to all deliverables of this SOW. b. Security Instructions: Access to classified information is not required for this SOW. c. Place of Performance: Silver Legacy Hotel, Reno NV ` d. Send Invoices: HQ AFSVA/SVPY 10100 Reunion Place San Antonio Texas 78216-4138 PERIOD OF PERFORMANCE: 12 OCT 04. Award will be made using Simplified Acquisition Procedures, therefore the evaluation procedures at FAR 13.106-2 will be used. A firm-fixed price award will be made to the responsible offeror submitting the quote that provides the best value to the Government. Interested parties capable of providing the above must submit written price quote to include discount terms, cage code, and firm?s evidence of their ability to perform. All responses will be considered, but must be submitted on the requirement included in this synopsis. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database, NO EXCEPTIONS. To register, you may apply at http://www.ccr.dlsc.dla.mil. All responsible sources may submit a quote. Quotes are due not later then 4:30 P. M., Central Standard Time (CST), 13 Sep 2004, 2:30 P.M. (CT) and should reference the solicitation listed above. Offerors must include with their quote, a completed copy of the provisions at FAR 52.212-3, Offerors Representations and Certifications-Commercial Items, 52.219-1, Small Business Program Representations, and 52.219-1 Alternate I, Small Business Program Representation and DFARS 252.212-7000, Offeror Representations and Certifications?Commercial Items. Responses to this notice shall be provided in writing via e-mail to 2LT Terence Balmaceda or TSgt Susan Brett-Farnsworth at terence.balmaceda@randolph.af.mil or susan.brett-farnsworth@randolph.af.mil. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following clauses also apply: 52.204-7 Central Contractor Registration; 52.212-4 Contract Terms and Conditions-Commercial Items. Paragraph (c) of this clause is tailored as follows: (c Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties, with the exception of : (1) Administrative changes such as changes in the paying office, appropriation data, etc. (2) No cost amended shipping instructions (ASI) if?(A) The ASI modifies a unilateral purchase order, and (B) The contractor agrees verbally or in writing, (3) Any change made before work begins if?(A) The change is within scope of the original order, (B) The contractor agrees; ( C ) The modification references the contractor?s verbal or written agreement. 52.212-5, Contract Terms and Conditions ?
