Description

This is a combined synopsis/solicitation for sole source commercial items prepared in accordance with the format in Subparts 12.6 of the Federal Acquisition Regulations, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a written solicitation will not be issued. This announcement incorporates commercial item provisions and clauses in affect through Federal Acquisition Circular 01-24. The North American Industrial Classification System code is 339112 Surgical and Medical Instrument Manufacturing, with a small business size standard of 500 employees. The Government intends to award a Firm Fixed Price purchase order under Simplified Acquisition Procedures (SAP) to Healthpoint Technologies Inc for their CIM-Healthpoint Counseling Information Modules/Units. Any parties that believe that they can satisfy the requirement below should send written documentation to Bonnie Pijanowski at VA Medical Center, 3495 Bailey Avenue, Buffalo, NY 14215 (mail routing symbol 90). All qualifications must be received no later than Tuesday, September 14, 2004. 1. The Healthpoint units are intended for our veteran and staff use and provides: * Major health topics that is clearly outlined on the main screen to help the user find the exact information that is sought. * A fully searchable database of over 2400 health and health-related topics updated quarterly. * An easy to use interface, clearly listing all the associated topics for a particular search. * Topics with video format that are clearly marked. * Pictures, video and audio help to clearly explain each topic. There are almost 700 instructional videos and pictures included in the database. * Topics that are grouped together in common areas to help the user find exactly what information they require. * A fully featured Back Office section to assist the staff to get the most from the Healthpoint System. * A comprehensive reporting/usage section that helps the staff determine the topics that are the most popular with the veterans and measure usage. *Customized Printouts. 2. Technically these Healthpoint units contain: * Hyperlinks to topic. * A handicap accessible molded plastic chassis of Euro-Kiosk design that is 3 ft 10.6" in Height/23.6"Width/ 23.5" Deep Future options important to V2 Strategic PHE CBOC Plan provides for an option for a Desktop Counseling Information system that is 17.6' Height/16.6" Width/ 10" Deep. This option will provide for personal counseling. It is small and lightweight making it ideal for easy positioning. 3. Software: * Operating System: Windows Professional 2000 operating Healthpoint program and the Healthpoint information database *Provides a 4-year Software Subscription with software updates provided four times per year. *Allows for a 4-year one-time payment for Software Updates. 5. Hardware Warranty * Includes a 3-year warranty on all parts. This is a component replacement plan for 3 years from the date of installation. This plan includes phone and/or e-mail troubleshooting and if the part is not functioning will send a replacement part. The customer agrees to install the component and return the damaged component at Healthpoint's cost. *Includes toll free use of the Customer Service Support Center. * State of the art equipment * Advance Status Reporting
