Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation RFQ 515-06-05 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-24. This is under the North American Industrial Classification System (NAICS) code 334220 and Small Business Size Standard of 750 employees. The procurement is for providing pagers and service for the Department of Veterans Affairs Medical Center, Battle Creek, Michigan for the period October 1, 2004, through September 30, 2005,with one renewal option year. The plan is to include paging service for 320 digital pagers and7 alpha-numeric pagers with option to add more during the contract period as necessary. Coverage must be state-side for the State of Michigan. Contractor must provide sales representative assigned to this site and be available to process requests Monday through Friday during normal working hours and provide emergency contact for night and weekend emergency needs. Contractor is to provide six pre-numbered pagers at the site that may be given out for emergency use. Monthly charges for these pagers will not begin until they are issued to Medical Center staff for use. A supply of six ?spare? pagers with no pager number assigned is to be provided. Monthly charges for these pagers will not begin until they are put into use. Quote is to address a cost of replacement pagers for those that may be lost during the contract period. All equipment must be new and any services requested must be delivered within two business days after request. Contractor must have all equipment programmed and ready to issue on site for conversion within one week after contract award. Contractor must be on site to assist with conversion process and to provide any necessary training needed for new equipment. Mandatory fees and any additional charges associated with the contract must be addressed in quote. Detailed coverage map as of the date of submission must accompany quotation. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition. Commercial Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The following FAR and VAAR addendums are incorporated into FAR 52.212-4 as follows: 52.217-8 (which provides that the Government may extend may exercise within 15 days notice, in writing); 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer), 852.237-70 (Michigan), 852.270-1 and 852.270-4. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.225-16, 52.232-34.
