Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quote (RFQ) under the number FA4452-04-Q-A338. This document and incorporated provisions and clauses are those in affect through Federal Acquisition Circular 2001-24. The North American Industry Classification System (NAICS) code for this acquisition is 339931. Small business size standard is 500 employees. The AMC Contracting Flight plans to issue a purchase order under the test program described in FAR 13.5 for the contractor to accomplish the following: The contractor shall make and deliver AF Family Member Programs ?N ?Shape? Youth Fitness Initiative Program bear as described below: 6 inches in height from seated position with legs extended; Body constructed of high quality Velboa furry polyester material; Body color red with white snout, white inner ear and white paw pads; Nose, smiling mouth and digits of paw pads stitched in black thread; Black plastic eyes with lighter shade brown outer rim; Body stuffed with thermoplastic beads or other suitable stuffing; 2005 embroidered on bottom of left leg paw pad (black); American flag embroidered on bottom of rig paw pad; Air Mobility Command Shield embroidered on left chest; Polyester furry fiber golf club attached to right hand paw pad; Polyester golf ball attached to left hand paw pad; Bead chain around neck w/AF logo miniature dog tag (blue/white); White visor sun hat attached to head; N Shape logo embroidered on sweat band of visor hat (black/red);Wrap around style sunglasses attached to face; Opening Hang tag on left ear w/N Shape Logo on cover; AMC Fitness Bear printed on inside (left) of hang tag; Services Family Member Programs printed on inside (right) Vertical. Information shall be provided in a format that reflects the price per item and total contract price. Total cost shall include shipping and handling. Delivery will be No Later Than 60 days after receipt of order to 12 AMC bases. A quantity of fifty (50) each will be shipped to Andrews AFB, Charleston AFB, Dover AFB, Fairchild AFB, Grand Forks AFB, MacDill AFB, McConnell AFB, McGuire AFB, Pope AFB, Scott AFB, Travis AFB, and to AMC/A7SCY (Lynda Miller). The provision at 52.212-1, Instruction to Offerors ? Commercial, applies to this acquisition. Award will be made to the lowest price technically acceptable offeror. Offerors are required to submit a completed copy of FAR 52.212-3, Offeror Representations and Certification ? Commercial Items, with their offer. The clause at 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders ? Commercial Items, applies to this acquisition, with the following clauses: 52.222-21 Prohibition of Segregated Facilities (FEB 1999); 52.222-26, Equal Opportunity (E.O. 11246); 52.222-35, Affirmative Action for Special Disabled Veterans, Veterans of the Vietnam Era and Other Eligible Veterans (38 U.S.C. 4212); 52.222-36, Affirmative Action for Workers with Disabilities (29 U.S.C. 793); 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era and Other Eligible Veterans (38 U.S.C. 4212). 52.232-33, Payment by Electronic Funds Transfer - Central Contractor Registration (31 U.S.C. 3332). The clause at 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (Jun 2003) applies to this acquisition. This is a DO rated order under the Defense Priorities and Allocations System (DPAS). The DFARS clauses 252.204-7004 Alternate A, and 252.232-7003 Electonic Submission of Payment Requests (Jan 2004). Wide Area Work Flow procedures will be used for invoicing and acceptance.
