- <DESC> This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/ ). The RFQ number is N00140-04-Q-1054. This solicitation documents and incorporates provisions and clauses in effect through FAC 2001-24 and DFARS Change Notice 20040625. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acqnet.gov/far and http://www.acq.osd.mil/dpap/dfars/index.htm . The NAICS code is 541219 and the Small Business Standard is7 million. This is a competitive, unrestricted action in support of the National Naval Medical Center, Bethesda, MD for the digitization of Emergency Room Treatment Records. The Fleet Industrial Supply Center Norfolk Detachment Philadelphia requests responses from qualified sources capable of providing: 0001, The digitization of 84,000 Emergency Room Treatment Records. It is anticipated that each emergency room treatment record will have on average four to five pages. Treatment records will consist of paper based and may include other media to include such items as kodachrome photographic slices, Polaroid patient pictures, radiology films and other colorized media both for diagnostic and treatment. The work to be performed will be in accordance with the statement of work as follows: The National Naval Medical Center (NNMC) Bethesda, Maryland is seeking assistance in managing its workspace by cutting down on the space utilized by boxes of Emergency Treatment Room (ETR) sheets. NNMC treats approximately 1000 patients a month in the Emergency Room. NNMC is required to keep ETRs on file for seven (7) years. Each box of ETRs is indexed and placed in Building 12 for filing. The medical reports described in this solicitation are irreplaceable medical information and must be safeguarded at all times. The contractor must understand and agree, that medical records of a patient are strictly confidential. The contractor will not maintain, in any form, any sensitive or patient identifying data and shall comply with Public Law 93-578, Privacy Act of 1974, The Comprehensive Alcohol Abuse and Alcoholism Prevention, Treatment and Rehabilitation Act of 1972, the Drug Abuse Office and Treatment Act of 1972, as well as other statutes regarding confidentiality of patient information. The Contractor and all employees must acknowledge understanding that this contract is subject to the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996. The contractor will certify compliance with the act by each signing a Business Associate Agreement (BAA). The successful quoter will (a) Provide industry leading, best practices based services expertise, in digitizing ETRs in a PDF format onto Compact Discs. (b)Compact Discs will be supplied by contractor. ( c ) Each CD will be accompanied with an index sheet for easy identification of which patients are on each CD, (d) ETRs will be sorted by month and year, and indexed by last name, first name, (e) Each index will include Last name, First name, FMP, Social Security Number, and date of visit, (f) Contractor will be responsible for transporting ETR boxes off the compound, digitizing the sheets at an off-site secure location within the United States reassembling re-boxing and then returning the ETR boxes back to NNMC Bethesda. Contractor will be responsible for Transporting ETR boxes out of room and into vehicle with the contractor?s equipment, (g) Each box will be inventoried and signed for by the contractor and verified by Government POC, (h) Contractor will be responsible for supplying a copy of an ETR during the archiving project within 24 hours of a Government?s request, (i) CD and index will be returned to NNMC within a hard protective case and binder for safe keeping, (j) Contractor will be responsible for providing names of personnel needed to access base for removal of records, (k), CDs will not contain multiple years on one CD, each CD will be limited to a month and year. Additional Requirements, quoter will provide an original and back up CD of all information. Each CD will be checked and certified by contractor for verification of content. Each CD will be signed for contents by Contractor management staff. Phone and email inquiries by the Government to the contractor will be responded to during the same working day, unless initiated after working hours. A phone answering service or machine utilized by the contractor during normal working hours will not constitute an answer to the DoD and will require a timely individual response. The contractor will perform all work in accordance with all applicable government regulations and directives. The successful quoter will provide an individual point of contact to the Government. This individual will assist in addressing government requirements; provide project support. The Government will provide the successful quoter with (a)Government Business Associates Agreement to be signed by contractor to uphold HIPAA requirements and the safekeeping of personal health information, (b) A Primary and Secondary POC , ( c) Contractor access to the base and to the building/room where records are held. Quoters are invited to view a sample of records to be scanned, appointments to view a case of ER treatment record samples must be made with Lt. Blake Allison who may be reached at 301-295-6620. Appointments can be made between 0900 and 1600 starting 2 September through 7 September. Performance of this requirement will begin date of award. The performance schedule for the work to be performed under this requirement is approximately 21,000 ETRs per month until all 84,000 ETRs are completed. In the event a record is needed by NNMC during the time said record is at the quoters facility a copy shall be made and either emailed, faxed or expressed mailed back to NNMC. Responsibility and Inspection: unless otherwise specified in the order, the supplier is responsible for the performance of all inspection requirements and quality control. The following FAR provision and clauses are applicable to this procurement: 52.212-1, Instructions to Offerors - Commercial Items; 52.212-3 ALT I, Offeror Representations and Certifications - Commercial Items; 52.212-4, Contract Terms and Conditions - Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (7), (8), (9), (13), (14), (15), (16), (17), (18),(19),(29). Quoters are reminded to include a completed copy of 52.212-3 ALT I with quotes. All clauses shall be incorporated by reference in the order. Additional contract terms and conditions applicable to this procurement are: 252.212-7000, Offeror Representations and Certifications - Commercial Items and 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items including 52.203-3 entitled Gratuities, and 252.243-7002 entitled Request for Equitable Adjustment. It is noted that FAR clause 52.222-41 entitled Service Contract Act of 1965 along w/ wage determination # 94-2103 Rev 33 is incorporated into this RFQ by reference. All responsible sources may submit a quote which shall be considered by the agency. 52.212-2, Evaluation - Commercial Items is applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors - a combination of price and past performance, with past performance being considered more important than price. Central Contractor Registration (CCR). Quoters must be registered in the CCR database to be considered for award. Registration is free and can be completed on-line at http://www.ccr.gov/ . All quotes shall include price(s), FOB point, a point of contact, name and phone number, GSA contract number if applicable, business size, and payment terms. Quotes over 10 pages in total will not be accepted by facsimile. Each response must clearly indicate the capability of the quoter to meet all specifications and requirements. The quoter shall submit, as part of its quote, information on previously performed contracts or on-going contracts that are similar in scope complexity and magnitude to the statement of work/item description in the solicitation performed for Federal, State or local Governments, and for commercial firms. Information shall be provided on your most relevant and will not exceed three sources. The information submitted shall include a contract number, the total dollar value, and period of performance of the contract. A POC knowledgeable about your performance under that contract. A description of the services performed under that contract, the # of and type of quality, delivery issues associated with that contract and how they were resolved, and any other information the quoter considers relevant to past performance of the referenced contract. The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government. Quoters lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance. However, the quote of a company with no relevant past performance history, while not rated favorably or unfavorably for past performance, may not represent the most advantageous quote to the Government and thus, may be an unsuccessful quote when compared to the responses of other quoters. The quoter should provide the information requested above for past performance evaluation, or affirmatively state that it possesses no relevant directly related or similar past performance. The assessment of the quoter?s past performance will be used as a means of evaluating the relative capability of the responder and other competitors to successfully meet the requirements of the RFQ. The Government will give greater consideration to the contracts that the Government feels are most relevant to the RFQ.
