Description

VA Minneapolis, MN has a requirement for a contractor to provide under Request for Proposal #RFP 618-16-05 entitled ?Transurethral Microwave/Laser Therapy Services for BPH? specialized medical resources services for transurethral microwave therapy and laser therapy for Benign Prostatic Hyperplasia (BPH) to VA beneficiaries to be furnished by the contractor at the VA. Contractor to provide services on a transmobile basis using a treatment/procedure room furnished by the VA. Services are to be comprehensive with the contractor providing the microwave delivery system, the disposable microwave antenna, the prostate microwave tray including most supplies needed for the procedure, and the clinical urological technician who sets up and monitors the microwave delivery system. VA will conduct the procurement according to a combination of FAR Part 12 and Part 15, and under Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. NAICS Code 621999 applies for this procurement and the small business size standard is $8.5 million. Contract is for a base one-year period with options to renew for two additional twelve-month periods. Solicitation opens on September 22, 2004. Solicitation packages will not be faxed or e-mailed.
