- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is set-aside for small business only. The NAICS Code for this synopsis/solicitation is 235520, Size Standard is $12 million. PR F1GPL141910100 is issued as a Request for Quote. This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular Fac 2001-24, Effective 19 Jul 2004. The statement of work below describes the government requirement. The Government intends to award one contract line item number (CLIN) for the following: removing any and all old carpet and cove molding and provide all labor, materials, tools, equipment, and transportation to purchase and install new carpet, cove base, and transitions in Building 1124. The desired new carpeting should be Shaw?s Pop Quiz Carpet on Ultraloc MPC backing with one (1) foot border of Shaw?s Design Series. Color TBD. Cove Base should be four (4) inch 1/8 inch Johnsonite rubber base. Color TBD. The Contractor may submit through the Contracting Officer a product or equipment that is equal or better than that specified. All carpet must meet the requirements as outlined in FC 3-600-01 and NFPA 101 for flame spread and smoke development. All products and equipment shall comply with the Government?s Buy American Act provisions. STATEMENT OF WORK Replace Carpet, Building 1124 SCOPE: The work covered in this Statement of Work consists of removing any and all old carpet and cove molding and provide all labor, materials, tools, equipment, and transportation to purchase and install new carpet, cove base, and transitions in Building 1124. 1. LOCATION: Building 1124, 50 Patrick Loop, Hanscom AFB, MA. (Room No.) 2. SUPERVISOR: The Contractor shall have a supervisor on site at all times during removal and installation. 3. NORMAL WORKING HOURS: Monday thru Friday, 0700 to 1600 (excluding weekends and holidays. Any changes to these hours shall be coordinated with the site POC, Mr. George Trainer or his designated representative. The phone number for Site POC is (781) 377-9925. 4. PERIOD OF PERFORMANCE: Shall be approximately three weeks from the date of contract award with the exception of long lead items which shall be verified in writing from the supplier to the Contracting Officer. 5. NOTIFICATION OF COMMENCEMENT: After contract award, the contracting officer, with coordination from the Site POC, will notify the contractor at least 72 hours prior to the start of the project. The contractor shall notify the Site POC prior to the start of the work. 6. MEASUREMENTS: Contractor shall verify all measurements in the field and report significant discrepancies to the Contracting Office. 7. CLEAN UP: Contractor shall be responsible for daily clean up of the work site, more often as necessary to maintain a clean and safe working environment. 8. APPEARANCE OF WORK SITE: Construction Dumpsters need to be coordinated with Del-Jen (Base CE?Customer Service at 781-377-3383 on Hanscom AFB)as they are not to be overflowing. Trash, coffee cups, lunch bags, etc., from a work site need to be disposed of properly by contractor employees. If the contractor needs to put up any signs, the signs must be the color brown as the basic color. Yellow tape should not be used to tape off work areas. Contractor storage areas need to be kept orderly while the contractor is using them. At the end of the contract period the area should be clean. Orange cones, if used, should be moved immediately when purpose of use is accomplished. The cones are to be picked up and put back down daily until they can be removed from the work site. 9 . SAFETY: The Contactor is solely responsible for compliance with the Occupational Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standard 29 CFR 1910, 1926, and the protection of their employees. It is the contractor?s sole responsibility to make certain that all safety requirements are met. Additionally, the contractor is responsible for the safety and health of all sub-contractor employees. The contactor shall immediately report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job to the contracting officer. Additionally, the contractor is responsible for securing and impounding evidenceof damages until the CO has been notified and CO has determined the course of action to be undertaken. , It will be the contracting officer?s responsibility to notify the ESC Safety Office. The Air Force?s interest is to protect all personnel working in or around construction operations, and all Air Force equipment/property on Air Force installations. In the event an unsafe condition is discovered with Air Force provided facilities or equipment, the contractor will notify the contracting officer who in turn will immediately notified the ESC Safety Office. Air Force Safety, Fire Protection, and Bioenvironmental Engineering officials may periodically enter a contractor?s workplace to verify working conditions of the Air Force personnel, provided the contracting officer authorizes such action. If an improper procedure or unsafe condition exists, which places AF personnel or equipment in jeopardy the contracting officer will be notified and the situation corrected. Department of Labor, OSHA inspectors are authorized right of entry to inspect any place of employment operated by an Air Force contractor. They are, for the most part, ?NO NOTICE,? inspections. NOTE: Notify the ESC Safety Office, 781-377-5135, if an OSHA inspector visits your site unescorted by an Air Force Safety Technician. If an Air Force inspector observes a potential OSHA violation, he/she will report the violations to the contracting officer, who will then notify the contractor. 10. PRODUCTS/MATERIALS: The Contractor shall follow the written manufacturer?s instructions and procedures for all products and equipment specified in this Statement of Work. The desired new carpeting should be Shaw?s Pop Quiz Carpet on Ultraloc MPC backing with one (1) foot border of Shaw?s Design Series. Color TBD. Cove Base should be four (4) inch 1/8 inch Johnsonite rubber base. Color TBD. The Contractor may submit through the Contracting Officer a product or equipment that is equal or better than that specified. The Contracting Officer reserves the right to disapprove a product submitted as ?equal or better? based on quality, composition and application comparison. All carpet must meet the requirements as outlined in FC 3-600-01 and NFPA 101 for flame spread and smoke development. All products and equipment shall comply with the Government?s Buy American Act provisions. The Contractor may request a copy of the Buy America Act from the Contracting Officer. 11. DESCRIPTION OF WORK: a. The contractor shall schedule the carpet removal and installation with site POC George Trainer. The carpet work in the building shall starton a weekday and completed by the Friday of that same week (exact day to be determined at a later time). b. Underneath the old carpet is asbestos tile flooring, which shall not be damaged or removed by the contractor. New carpet and moldings will be applied over the old tile floor. Damage is defined herein as breaking, sanding, grinding, cutting or abrading the tile flooring. If damage does occur to the tile floor, the POC, George Trainer shall be immediately notified at (781) 377-9925. Work on the flooring shall stop until the POC has directly observed the damage and either given an official ?Stop WORK? order verbally to the contractor or permitted the contractor to proceed. The site POC, George Trainer, shall supervise the removal of any small amounts of tile that have been disturbed by the contractor. Should extensive damage occur to the floor tile, all work on the flooring shall stop and both the Contracting Officer, Mona Joseph-Scott at (781) 377-2113, and the work site POC, Mr. Trainer, at above phone number, shall be notified. c. Mr. Raymond Salvas of the Environmental Safety Office (781) 377-3871 shall be contacted by the site POC if excessive damage occurs to the tile floor for an assessment as to removal of the tile. A licensed Asbestos Abatement contractor may be needed for removal of the remaining carpet and tile. Such contractor must work IAW Hanscom AFB Asbestos Abatement Specifications. d. The contractor shall remove and dispose of all existing carpet and cove base in a manner that results in no damage to any portion of the wall or floor. See paragraphs (b) and (c) of this section for removal of any tile flooring and the necessity of working with the POC on this job in doing so. The contractor is responsible for repair of any damage that does occur. This includes, but is not limited to, patching and painting of walls, repair of door thresholds, etc. If any doors do not open and close properly as a result of installation or height of the new carpet, the contractor shall be responsible to remove the door from its hinges, sand the door to allow the door to swing freely, and rehang the door. d. The contractor shall properly fill all cracks, spalls, and depressions in the sub-floor with a material that will be compatible with the manufacturer?s recommended procedures for the type of carpet and adhesive to be used. e. The contractor shall properly clean the entire floor area to be covered after carpet installation. f. The carpet and cove base shall be installed in the specified areas as depicted in the attached floor plan. The carpet shall lay flat with no kinks or wrinkles. g. Upon completion of the installation, the contractor shall clean all areas to ensure all dirt, material waste, and other debris are removed and disposed of properly. 12. DISPOSAL: The contractor shall remove all waste and debris from Building 1124 and dispose of properly in accordance with all Local, State and Federal Codes. The period of performance shall be approximately three weeks from the date of contract award with the exception of long lead items which shall be verified in writing from the supplier to the Contracting Officer. A site visit has been scheduled for Thursday 2 Sept 04 at 11:00 AM where all interested offerors will be provided with drawings of the areas covered by this request. All interested offerors shall contact Sara Denis with the names of all attending the site visit by 4:00 PM EST Wednesday 1 Sep 04 for access to the installation. Quotations are due the following Tuesday 7 Sep 04 no later than 2:00 PM EST. Quotations that do not include prices for all requirements will not be considered. The following provisions and clauses apply to this procurement: FAR 52.212-1, Instructions to Offerors-Commercial, FAR 52.212-2, Evaluation?Commercial Items: Award will be made to the responsible offeror whose offer is considered to be most advantageous to the Government, considering technical acceptability, and price are of equal importance.
