- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91ZRS-04-R-0013 and is issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through F ederal Acquisition Circular 01-15. The Connecticut Army National Guard has the requirement for the publication of two thousand (2000) hardcover yearbooks, overall size 9 X 12 , approximately 200 colored pages, sewn textbook binding, rounded and backed w ith headbands. Silk screen/litho color applied or company designed 4 color/standard cover. Offer should be prepared to submit at least one previously published sample to USP&FO P&C, attn: Karen Havens, 360 Broad Street, Hartford, CT 06105. The followi ng factors shall be used to evaluate offers: 1. Past Performance 2. Price. Past Performance is equal to price.
