Description
This notice is for informational purposes only. The Centers for Disease Control and Prevention intends to issue a sole source purchase order to National Association of Medical Examiners, 430 Pryor Street, SW, Atlanta, GA 30312. The National Violent Death Reporting System (NVDRS) is a population-based surveillance system that captures information on violent deaths in participating states. The purpose of this contract is to collaborate with a national organization that represents medical examiners and/or coroners to develop strategies for improving data collection from Medical Examiners/Coroners. The deliverables for this contract will be to provide expert advice in regard to working with state medical examiners and/or coroners to access Medical Examiner data by August 31, 2005. Serve as a liaison with Medical Examiners and or Coroners in the National Violent Death Reporting System (NVDRS) funded states. This includes gaining feedback from Medical Examiner/Coroners regarding the impact of NVDRS on operational activities and costs by June 1, 2005. Develop a set of recommendations for gaining input from coroners regarding violent death reporting data. The recommendations would include a process for increasing Medical Examine/Coroners interaction by August 31, 2005. Send one Medical Examiner or Coroner to the NVDRS implementation training to provide consultation on working with Medical examiner or Coroner offices by December 1, 2004. Explore the possibility of an additional ME/Coroner set of variables that can be added to the electronic death certificate by August 31, 2005. The National Association Medical Examiners in Atlanta, GA is the only known source that can provide these services. Interested vendors capable of furnishing the government with the report in this synopsis should submit two (2) copies of a quotation to the below address. The quotation shall include the itemized price, prompt payment terms, Taxpayer Identification Number (TNN), Dun & Bradstreet Number (DUNs), and descriptive literature of the quoted. The quotation must reference Solicitation number 2004-Q-11819. Quotations will be due fifteen (10) calendar days from the publication date of this synopsis or August 30, 2004, by 8:30 a.m. local time, whichever is later. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Linda T. Robinson. Email quotations will only be accepted if dated and signed by an authorized company representative to: ltr1@cdc.gov. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. Quotes will be due August 30, 2004, by 8:30 AM EST at the Centers for Disease Control and Prevention; Facilities and Construction Management Branch; Attn: Linda T. Robinson; 2920 Brandywine Road, Room 3000, Atlanta, GA 30341. Facsimile quotes will be accepted at 770-488-2847. RFQ# 2004-Q-11819. Attn: Linda T. Robinson, shall be clearly marked on the envelope and/or facsimile cover page. All responsible sources that can meet the above requirement may submit a quote, which will be considered by the Agency. The Central Contracting Registry is a central repository of all Contractors who wish to do business with the Federal Government. All vendors who wish to bid on the requirement MUST be registered and validated in the CCR prior to award of this Simplified Acquisition. Vendor who submit a quotation are showing that they are registered or plan to do so. Vendors who are not registered will not be considered for award.
